Retrospective Analysis Of The Correlation Between Course Of Chronic HBV Infection And TCM Constitution

Objective: By collecting the patients with chronic HBV infection in different course of collecting and analyzing the constitution of traditional Chinese medicine,to observe the progression of chronic hepatitis b virus infection on the influence of the constitution of traditional Chinese medicine,discusses the natural course of chronic HBV infection and the correlation of TCM constitution,looking for different constitution may represent the progression and prognosis of,for clinical pathology constitution basis for syndrome differentiation and treatment of chronic HBV infection.Methods: Collect and analyze the complete clinical data of patients who were hospitalized and diagnosed with chronic HBV infection at the Ninth Hospital of Nanchang,Jiangxi Province from October 2017 to October 2019,using the "Guidelines for the Prevention and Treatment of Chronic Hepatitis B(2019 Edition)" as the standard.According to the inclusion and exclusion criteria,a total of 301 patients were selected as the subjects of this study.According to the different course of chronic HBV infection,they were divided into HBV carrier group(96 cases),chronic hepatitis B group(106 cases),hepatitis B cirrhosis group(58 cases),and hepatitis B liver cancer group(41 cases).Compare the correlations of the four groups at the first diagnosis.Clinical data,including gender,age,hepatitis B E antigen(HBe Ag),total bilirubin(TBIL),alanine aminotransferase(ALT),aspartate aminotransferase(AST),albumin(ALB),hepatitis B viral load(HBV)-DNA)and TCM physique type,analyze the factors that may affect the natural course of chronic HBV infection and the correlation between TCM physique.The measurement data is expressed as x±S,and the counting data adopts the 2-test,and P≤0.05 is considered to be statistically significant.Results:(1)234 male patients and 67 female patients were included in the study,with a male to female ratio of 3.49:1.(2)The patients included in the study were between 15 and 75 years old,with an average age of(39.71±13.92)years old.There were 159 cases of≤40 years old,accounting for 52.82%;115 cases of 41 to 59 years old,accounting for 38.21%;27 cases ≥60 years of age,accounting for 8.97%.(3)The TCM physique types of the HBV carrier group are mainly calm(73.96%),Qi deficiency(8.33%),and Qi stagnation(6.25%).The TCM physique of the chronic hepatitis B group is mainly moist heat(27.36%),Peaceful quality(16.98%),Yang-deficiency quality(16.04%),Qi-stagnation quality(13.21%).The TCM constitution types of the hepatitis B liver cirrhosis group are mainly Yang-deficiency quality(29.31%),damp-heat quality(15.52%),and peace quality(15.52%),the TCM constitution types of the hepatitis B liver cancer group are mainly Yang-deficiency(34.15%),Qi-deficiency(12.20%),damp-heat(12.20%).There are significant differences in TCM physique of patients with chronic HBV infection in different courses,and the difference is statistically significant(P≤0.05).(4)The traditional Chinese medicine physique of patients with chronic severe hepatitis B is mostly damp-heat,calm,and phlegm-damp.Male patients with chronic HBV infection are more likely to develop severe hepatitis.(5)There are obvious differences in the TCM constitution types of patients with hepatitis B cirrhosis of different genders.The TCM constitution types of male patients are mostly damp-heat,yang deficiency,and peaceful constitution,while the TCM constitution types of female patients are yang deficiency and qi.Stagnation and Qi deficiency were more common,and the difference was statistically significant(P≤0.05).(6)There are significantly more male patients in the hepatitis B liver cancer group than females.Among them,there are significant differences in medical constitution types in different age groups.The traditional Chinese medical constitution types of young and middle-aged liver cancer patients are more common in phlegm-dampness,qi deficiency,and qi stagnation.The types of constitution of traditional Chinese medicine of elderly patients are mostly yang-deficiency type and damp-heat type,the difference is statistically significant(P≤0.05).Conclusion:(1)There are far more men with chronic HBV infection than women,and men are more likely to be infected with HBV than women.(2)The natural course of chronic HBV infection is closely related to the age of the patient.The course of the disease deepens with age,and the condition of middle-aged and elderly patients is more serious.(3)There are significant differences in the types of TCM physiques of patients with different courses of chronic HBV infection.The progress and prognosis of the disease can be predicted to a certain extent by identifying the changes in physique.
