Correlation Between Vaginal Infection And HPV Infection And The Study Of TCM Constitution And Syndrome Types

Objective: through examining the microenvironment and HPV in patients with the main complaint of "vaginal itching,vaginal dryness and pain,vulva discomfort and abnormal secretion",to analyze the relationship between vaginal infection and HPV infection,to study the distribution of TCM constitution and syndrome types,to find out the susceptible population of vaginal infection and HPV infection,and to provide new strategies for the prevention and treatment of vaginal infection and HPV infection.Method: 483 patients with various main complaints of "vulva discomfort and abnormal secretion" and other complaints from the gynecological outpatient department of the Second Affiliated Hospital of Tianjin University of TCM from February 1st,2019 to December 1st,2019 were examined for vaginal microenvironment and cervical HPV virus,and filled in General Condition Questionnaire and Self-check Chart of TCM Constitution Classification and Judgment.After collation,the collected data were recorded in Microsoft Excel and analyzed by SPSS 19.0.Results: 1.The results of microenvironment examination: of the 483 patients,4 cases(0.8%)with normal vaginal microenvironment,129 cases(26.7%)of dysbiosis without definite infection,350 cases(72.5%)of vaginal infection with definite pathogens.The highest frequency of simplex infection was 111 cases(38.7%)for VVC,and the highest frequency of mixed infection was 27 cases(42.9%)for intermediate type BV with VVC.2.The related factors of vaginal infection were sexual intercourse frequency for more than 3times / week,douche by oneself of more than 3 times / week and spicy diet.3.There was a statistical difference in the distribution of TCM constitution between the vaginal infection group and control group.There was a negative correlation between gentleness constitution and vaginal infection,and there was a positive correlation between PHlegm-dampness constitution and vaginal infection.There was no statistical difference in the other TCM constitutions.Among the patients with vaginal infection,cases with syndrome of dampness-heat pouring downward was the most,accounting for 52.6%,22.6% for spleen-deficiency syndrome,12.0% for damp-warm disease with syndrome of damp-heat and amassing poison,10.2% for syndrome of dual deficiency of liver and kidney yin and accumulation of dampness heat,2.6% for kidney-yang deficiency syndrome.There were statistical differences in the distribution of syndrome types between the two groups.There was a positive correlation between syndrome of dampness-heat pouring downward,syndromeof damp-heat and amassing poison and vaginal infection.4.The results of HPV examination: of the 483 patients,HPV(+): 162 cases with the HPV infection rate of 33.5%.Among them,120 cases(74.1%)were single HPV infection and 42cases(25.9%)were multiple HPV infection.Among 120 patients with single HPV infection,99 cases(82.5%)were high-risk HPV infection,15 cases(12.5%)were low-risk HPV infection,and 6 cases(5.0%)were suspected high-risk HPV infection.5.There was a correlation between vaginal infection and HPV infection,but there was no significant correlation between vaginal infection and single or multiple HPV infection,high-risk or low-risk HPV infection,etc.There was a positive correlation between BV and HPV infection(P < 0.01,OR = 2.335),while there was no correlation between other vaginal infections and HPV infection.6.There was statistical difference in the TCM constitution between vaginal infection with HPV infection and without HPV infection.There were significant statistical differences in the gentleness constitution,yang deficiency constitution and PHlegm-dampness constitution,while there were no statistical differences in the other TCM constitutions.There was a negative correlation between gentleness constitution(OR = 0.144)and HPV infection,and a positive correlation between yang deficiency constitution(OR = 2.239),PHlegm-dampness constitution(OR = 2.381)and HPV infection.Conclusions: 1.The incidence of vaginal infection was higher,accounting for 72.5% in patients with "vaginal itching,vaginal dryness and pain,vulva discomfort and abnormal secretion".Vaginal simplex infection is more common.VVC is not only the most common simplex infection,but also the most susceptible to mixed infection.2.The related factors of vaginal infection include frequent sexual life,douche by oneself and spicy diet.In daily life,we should keep regular sexual life,eliminate douche by ourselves and keep bland diet.3.Patients with vaginal infection are prone to HPV infection,especially BV is closely related to HPV infection.It is recommended to seek medical treatment actively with symptoms of vaginal infection,and clinicians should suggest patients with vaginal infection to perform HPV examination and cervical TCT examination.4.People with the syndrome of dampness and heat of PHlegm and dampness were more likely to cause vaginal infection.Among the patients with vaginal infection,people with the syndrome of dampness and heat of Yang deficiency were more likely to be infected with HPV.There was no statistical difference in TCM syndrome types.
