Objective:1.Observe the total effective rate of siziwenfei Decoction before and after treatment;TCM syndrome integral;6MWD;Serum IL-13,IL-1β、Objective to evaluate the clinical efficacy of siziwenfei Decoction in the treatment of AECOPD(phlegm dampness accumulation in lung syndrome)and explore its mechanism.2.method:Methods: 60 AECOPD patients were randomly divided into control group and treatment group,30 cases in each group.The control group was given conventional western medicine treatment;the treatment group was given siziwenfei Decoction in addition to western medicine treatment,with a cycle of 10 days.The total effective rate,6-minute walking distance,TCM syndrome score,serum IL-13,IL-1β and hsCRP were compared between the two groups before and after treatment.Methods: 40 healthy male SD rats were randomly divided into 4 groups,with 10 rats in each group.They were divided into blank group,model group,siziwenfei Decoction group and ambroxol group.Except for 10 rats in blank group,the other 30 rats were used to establish COPD model of phlegm dampness accumulation in lung syndrome by smoking,intratracheal instillation of lipopolysaccharide and intragastric administration of senna leaf.The pathological changes of lung and bronchus were observed by HE staining;The levels of IL-13 and MUC5 AC in BALF were detected by ELISA.result:clinical observation:1.The total effective rate of the treatment group was higher than that of the control group(P < 0.05);2.The treatment group was better than the control group in improving TCM syndrome score(P < 0.05);3.After treatment,the 6-minute walking distance of the two groups increased,but the improvement effect of the treatment group was better(P < 0.05);4.The levels of serum IL-13,IL-1β and hsCRP in the treatment and control groups were improved compared with those before treatment,but the improvement in the treatment group was more obvious(P < 0.05).Animal experiment:1.HE staining showed that the bronchial and lung tissues of COPD model group were consistent with the pathological changes of COPD;Compared with the normal control group,the bronchopulmonary tissue of each treatment group showed different degrees of inflammatory cell infiltration,lumen stenosis and alveolar wall rupture,but compared with the model group,siziwenfei Decoction group and ambroxol group,the above pathological manifestations were significantly reduced,and siziwenfei Decoction group had more significant effect.2.ELISA: compared with the normal group,the levels of IL-13 and MUC5 AC in the model group were significantly increased(P < 0.01);Compared with the model group,the contents of IL-13 and MUC5 AC in siziwenfei Decoction group and ambroxol group were decreased(P < 0.01).Compared with ambroxol group,the contents of IL-13 and MUC5 AC in siziwenfei Decoction group were lower(P < 0.05).Conclusions:1.Clinical observation: siziwenfei decoction combined with conventional western medicine treatment can more effectively reduce the airway inflammation,relieve the clinical symptoms and improve the quality of life of patients,which is worthy of clinical application.2.Animal experiment: siziwenfei decoction can reduce the pathological changes of COPD model rats with phlegm dampness accumulation in the lung,further reduce the secretion of MUC5 AC by reducing the content of inflammatory factor IL-13,so as to reduce airway inflammation and inhibit airway mucus hypersecretion,so as to achieve the purpose of prevention and treatment of COPD.
