ObjectiveThe core competence scale of cardiac nurses in the operating room was developed to test the reliability and validity of the scale,so as to provide a meaningful evaluation tool for quantifying the core competence level of cardiac nurses in the operating room.Methods1.Construct item pool:Domestic and foreign guidelines,expert consensus and relevant literature on scale compilation which relate to core competence of specialist nurses,operating room and cardiac specialist nurses,operating room sub-specialist nurses were searched by searching Chinese and English databases.Scale dimensions and item pool were initially constructed.Scale items were supplemented through semi-structured interview that focus group interview was used to deeply understand and explore the connotation of core competence of cardiac nurses in the operating room.Content analysis method was adopted,and Nvivo12.0 software was used to sort out and analyze interview data.2.Form the scale:Delphi expert letter consultation,pre-survey and entry screening were applied to screen the contents of the entry pool.The specific methods are as follows:(1)Delphi expert letter consultation:25 experts from 8 provinces and cities in China are invited to conduct two rounds of Delphi expert letter consultation to evaluate the dimension division of the initial scale and the contents of items.(2)Pre-survey:Convenience sampling method was adopted to select 20 nurses who took on the task of teaching cardiac surgery as the survey objects for pre-survey,so as to test whether the contents and language expressions of the scale items are easy to understand.(3)Item screening:A total of 423cardiac nurses in operating rooms from tertiary hospitals with cardiac surgery and cardiovascular hospitals nationwide were selected by convenience sampling method for cross-sectional survey.After screening the scale items,the formal scale of core competence of cardiac nurses in operating room was formed from April to June 2022.3.Reliability and validity test:A total of 478 nurses specialized in operating rooms in tertiary cardiac surgery hospitals and cardiovascular hospitals nationwide were selected as research objects by convenience sampling method from August to October 2022.Test the reliability and validity of the formal scale.The reliability test used internal consistency coefficient,split half reliability,retest reliability and combination reliability,and the validity test used content validity,structure validity,convergence validity and discrimination validity.Results1.Results of entry pool construction:A total of 22 effective literatures were obtained through literature research.The semi-structured interview based on the theory of core competence extracted a total of 5 themes,namely clinical nursing competence,interpersonal competence,leadership and management competence,professional development competence and personal traits.Form a pool of 51 entries with 5 dimensions.2.(1)Delphi expert Letter Inquiry:The effective recovery rates of the two rounds of correspondence questionnaire were 100%,and the expert authority coefficients were 0.88and 0.91 respectively.The importance scores of each item in the first round of correspondence questionnaire ranged from(3.96±0.94)to(4.78±0.52).The importance scores of the items in the second round of expert letter consultation ranged from(4.00±0.89)to(4.86±0.48).The variation coefficients of the items in the two rounds of expert letter consultation were all less than 0.25.The Kendall harmony coefficients of expert opinions were 0.540 and 0.628 respectively.After the two rounds of expert letter consultation,5items were added,and 3 items were deleted,2 items were merged,and 22 items were modified.At last,he revised scale includes 5 dimensions and 52 items.(2)Pre-survey:Respondents believed that the entry language was clear and easy to understand,which was suitable for evaluating the core competence of cardiac nurses in the operating room.(3)Item screening:6 items that did not meet the screening criteria were deleted according to the item screening criteria,including C7,C8,C12,C25,C27 and C45.The core competence scale was formed for cardiac nurses in the operating room,which contained 5 dimensions and 46 items.3.(1)Reliability results:Cronbach’sαcoefficient of the total volume table was 0.973,and Cronbach’sαcoefficient of each dimension was 0.939~0.974.The retest reliability of the total volume table was 0.998,the retest reliability of each dimension was 0.994~0.998,and the combination reliability of each dimension was 0.910~0.969.It indicates that the content reliability of the scale is good.(2)Validity results:The scale I-CVI was between0.809 and 1.00,the S-CVI/UA value was 0.913 and the S-CVI/ave value was0.942,indicating that the content validity of the scale was good.AVE values of the 5dimensions are all greater than 0.6,and the correlation coefficients among all dimensions are smaller than the square root values of AVE,indicating that the scale has good discriminative validity.Confirmatory factor analysis results illustratedχ~2/df=1.744、RMSEA=0.052、IFI=0.935、TLI=0.931、CFI=0.935,suggesting good reliability and validity of scale structure.ConclusionThe core competence evaluation scale of cardiac nurses in operating room finished in this study includes 46 items from 5 dimensions including clinical nursing competence,interpersonal competence,leadership and management competence,professional development competence,and personal trait.It has good reliability and validity,which can be used as an evaluation tool for core competence of cardiac nurses in operating room.
