Objective:By analyzing and comparing the differences in clinical indicators between the hyponatremia group and the control group,the severity of serum sodium,the corrected serum sodium group and the uncorrected serum sodium group,the correlation between hyponatremia and the clinical indicators of SCLC was statistically analyzed to further guide the treatment and prevention of SCLC complicated with hyponatremia.Methods:1.500 cases of SCLC patients diagnosed by menstrual histomathology from January2015 to January 2019 were selected as the research objects,followed up to January 2020,and complete data of 465 cases of follow-up were obtained.Clinical data of the patients(gender,age,smoking,BMI,CEA,NSE,PRO GRP,NEUT%,HB,ALP,CR,UA,GGT,AST,TNM stage,PLT,ALT,LDH,ALB,ALP,distant metastasis,pleural effusion or radiotherapy)were analyzed.2.According to the baseline serum sodium value at the time of diagnosis of SCLC,they were divided into hyponatremia group(n=232)and control group(n=233).According to the lowest serum sodium value during the course of disease,the patients were divided into four subgroups: normal group(n=181),mild hyposodium group(n=113),moderate hyposodium group(n=80)and severe hyposodium group(n=91).After one cycle of first-line chemotherapy,the patients were divided into two subgroups: sodium corrected group(n=77)and sodium uncorrected group(n=388).3.The differences of clinical indicators related to hyponatremia and SCLC in each group were statistically analyzed to analyze which clinical indicators were independent influencing factors of SCLC complicated with hyponatremia.Results:1.The NSE,Pro GRP,NEUT %,Hb,AST,GGT,ALP,Cr,UA,pleural effusion in SCLC combined with low sodium group were higher than those in control group,and the differences between groups were statistically significant(P<0.05).There were no significant differences in sample size,AGE,BMI,CEA,PLT,ALT,LDH,ALB,TNM stage and metastatic site between the two groups(P > 0.05).2.There were statistically significant differences among the four groups in terms of normal serum sodium,light,medium and recombination,gender,smoking,NSE,PRO GRP,NEUT %,Hb,LDH,Cr,UA,and pleural effusion(P<0.05).3.After first-line chemotherapy,there were statistically significant differences in NEUT %,Cr and UA between the sodium corrected group and the sodium uncorrected group(P<0.05).4.Logistic regression analysis showed that NSE,NEUT%,ALT,AST,LDH,UA and pleural effusion were independent influencing factors for SCLC complicated with hyponatremia.Conclusion:1.In patients with SCLC complicated with hyponatremia,there may be abnormal biochemical indicators including liver function dominated by AST,GGT and ALP,renal function dominated by CR and UA,blood routine dominated by NEUT% and Hb,tumor markers dominated by NSE and PRO GRP.2.In the course of treatment,tumor markers mainly NSE and Pro GRP,blood routine mainly Neut % and Hb,and renal function mainly Cr and UA will deteriorate with the severity of hyponatremia.3.Clinical indicators Neut %,Cr and UA were significantly recovered with the recovery of hyponatremia,other clinical indicators with a trend in quantity but no statistical significance.4.Retrospective analysis showed that NSE,Neut %,ALT,AST,LDH,UA and other biochemical indexes were correlated with SCLC complicated with hyponatremia,and further prospective studies should be designed to confirm their correlation,so as to provide more basis for the recognition and treatment of hyponatremia,and to optimize the prognosis of patients with SCLC complicated with hyponatremia.
