Changes Of Immune Function And Inflammatory Index Before And After Surgery In Patients With Gastric Adenocarcinoma

Objective:To observe the changes of T cell subsets,Nature kill cell(NK cells),B cells and neutrophil percentage(NEU%),lymphocyte percentage(LYM%),neutrophil to lymphocyte ratio(NLR)of patients with gastric adenocarcinoma before and after operation,and to discuss the effects of operation on immune function and inflammatory indexes of patients and their different stages.Methods:70 patients with gastric cancer who underwent surgical treatment in Shanxi Cancer Hospital from June 2018 to December 2020 were all diagnosed as adenocarcinoma by pathology,and they were divided into different clinical stages.All patients were consulted by electronic case data to find out the general situation of gender,age,smoking and drinking history,tumor location,and whether they were complicated with malignant diseases of other systems.T cell subsets,NK cells and B cells were detected by flow cytometry three days before operation and half a month after operation.NEU%,LYM%and NLR in peripheral blood were also detected.Blood samples from healthy people were collected as control,and SPSS23.0 software was used for statistical analysis and observation of the changes of various indicators.Results:1.The percentage of CD4~+,CD4~+/CD8~+,CD16~+CD56~+,CD45~+CD3~-CD19~-,LYM%in peripheral blood of patients with gastric adenocarcinoma group before operation was lower than that of healthy control group,and the difference was statistically significant(P<0.05).CD8~+,Treg cells,NEU%and NLR were higher than those of healthy control group,and the difference was statistically significant(P<0.05).2.The percentages of CD45~+CD3~+,CD4~+,CD4~+/CD8~+,CD16~+CD56~+,CD45~+CD3~-CD19~-and LYM%in the peripheral blood of gastric adenocarcinoma before operation were lower than those in the postoperative group,with statistical significance(P<0.05).Treg cell,CD8~+,NEU%and NLR were higher than those in the postoperative group,and the difference was statistically significant(P<0.05).3.The levels of CD45~+CD3~+,CD4~+,CD4~+/CD8~+in preoperative stage Ⅰ were lower than those in postoperative stage I(P<0.05),while CD8~+was higher than that in postoperative stage I(P<0.05),and there was no significant difference in other indexes(P>0.05);The levels of CD45~+CD3~+,CD4~+,CD4~+/CD8~+,LYM%in preoperative stageⅡwere significantly lower than those in postoperative group(P<0.05),and NLR was significantly higher than that in postoperative group(P<0.05).There was no significant difference in other indexes(P>0.05);The levels of CD45~+CD3~+,CD4~+,CD4~+/CD8~+,CD16~+CD56~+,CD45~+CD3~-CD19~-,LYM%in preoperative stageⅢwere lower in preoperative group than those in postoperative group(P<0.05),while CD8~+,Treg cell,NEU%and NLR were higher in postoperative group.4.With the development of TNM staging of gastric adenocarcinoma,the percentage of CD45~+CD3~+,CD4~+,CD4~+/CD8~+,CD16~+CD56~+,CD45~+CD3~-CD19~-and LYM%in peripheral blood decreased,while Treg cell,CD8~+,NEU%and NLR increased,while CD4~+in stage Ⅰ and Ⅱ increased.However,CD8~+ratio was lower than that in stageⅢ,CD45~+CD3~+in stage Ⅱ was higher than that in stageⅢ,LYM%in stage Ⅰ was higher than that in stageⅢ,and neutrophil percentage and NLR were lower than those in stageⅢ,with statistical difference(P<0.05).Treg cells had no statistical difference among the different stages(P>0.05).5.CD4~+,CD4~+/CD8~+and NK cells in peripheral blood of patients with stageⅢgastric adenocarcinoma after operation were lower than those in stage Ⅰ and stage Ⅱ,and the difference was statistically significant(P<0.05).CD16~+CD56~+in stage ⅡI was lower than that in stage Ⅱ,and the difference was statistically significant(P<0.05).Conclusion:1.There are significant differences in T cell subsets,NK cells,B cells and inflammatory indicators between patients with gastric adenocarcinoma and healthy people;2.The lower the expression of CD45+CD3+,CD4+,CD4+/CD8+,CD16+CD56+,CD45+CD3-CD19-,the higher the expression of CD8+and Treg cell,and the later the stage.NEU%and NLR can be used to evaluate the degree of inflammation and promote tumor proliferation;3.The later the stage,the worse the immune function and the higher the inflammatory index.Surgery can improve the immune function of patients and reduce the inflammatory reaction of the body.
