Objective: To explore the the association between single nucleotide polymorphism(SNP)of adiponectin(APN)and adiponectin receptor(Adipo R)gene and coronary heart disease(CHD),and to analyze the correlation between SNP and the severity of CHD.At the same time,the expression of serum adiponectin in healthy people and people with coronary heart disease was detected,and the relationship between serum adiponectin concentration and SNPs polymorphism was analyzed.In addition,the relationship between traditional risk(protective)factors and genotypes of coronary heart disease was further analyzed.The purpose of this study was to elucidate the relationship between single nucleotide polymorphisms of adiponectin and adiponectin receptor gene and coronary heart disease(CHD),so as to provide some theoretical basis for the prevention,treatment and diagnosis of CHD.Methods: From August 2018 to October 2020,263 patients with coronary heart disease were selected from the Department of Cardiology of the affiliated Hospital of Guilin Medical College,according to their medical history,clinical manifestations,ECG and coronary angiography,and 235 healthy controls were collected in the same period.The expression of serum adiponectin in CHD group and control group was detected by enzyme-linked immunosorbent assay(ELISA).The genotypes of four SNPs(rs1501299,rs3774261,rs266729 and rs7539542)in adiponectin gene were analyzed by polymerase chain reaction restriction fragment length polymorphism(PCR-RFLP)and direct DNA sequencing.Results: The median of serum APN concentration was 14.40(1.42-52.26)μg/ml in CHD group and 29.40(3.18-90.31)μg/ml in control group.Mann Whitney U test showed that the serum APN level in CHD group was significantly lower than that in control group,and the difference between the two groups was statistically significant(z =-8.909,P < 0.001).The genotype of rs266729 in CHD group was significantly different from that in control group(P < 0.001),but there was no significant difference in allele frequency between the two groups(P > 0.05),However,after adjusting for gender and age,the mutation homozygous GG genotype increased the risk of coronary heart disease compared with wild homozygous CC genotype(OR = 2.740,95%CI 1.109-6.768,P = 0.029).The relationship between the dominant and recessive models of rs266729 genotype and coronary heart disease showed that the mutation homozygous GG genotype carriers significantly increased the risk of coronary heart disease compared with the C allele carriers(CG + CC)(OR=2.156,95% CI 1.004-4.631,P = 0.049),and this effect still existed after adjusting for gender and age,and the significance was more significant(OR = 2.695,95%CI 1.110-6.540,P = 0.028),The GG genotype was 2.695 times more likely to have CHD than CG + CC genotype,and the rs266729 genotype was an independent risk factor for CHD.When rs266729 allele C mutation is G,it increases the risk of coronary heart disease.However,rs1501299 of APN and rs7539542 of Adipo R1 were not statistically significant in CHD group and control group(P > 0.05).There was no significant difference in serum adiponectin levels between the two loci,and the difference was not statistically significant(P > 0.05).Conclusions:(1)There is indeed a difference in the level of serum APN between patients with CHD and healthy people in northern Guangxi.The level of APN in the CHD group is lower than that in the control group.Hypoadiponectinemia is one of the risk factors of coronary heart disease.(2)The SNP rs266729(C > G)mutation of APN gene can significantly increase the risk of coronary heart diseasein the population of northern Guangxi,especially the homozygous GG genotype can be used as an independent risk factor for coronary heart disease.(3)There was no significant correlation between rs1501299 SNP of APN gene and rs7539542 SNP of Adipo R1 gene and coronary heart disease in the population of northern Guangxi.(4)The rs1501299 and rs266729 SNPs of APN gene and rs7539542 SNP of Adipo R1 gene were not significantly correlated with serum APN level in the population of Northern Guangxi,which could not be used as a predictor of APN level.
