Objective: To investigate the resilience and cognitive function of the elderly in extreme poverty in rural areas of Anhui province.This paper analyzes the relationship between resilience and cognitive function of the elderly in extreme poverty in rural areas,and provides a basis for formulating strategies to improve the cognitive function of the elderly in extreme poverty in rural areas.Methods: Cluster sampling was used in all 16 cities in Anhui Province,and 2 nursing institutions were selected from each city that were willing to participate in the survey.The Concord-Davidson Resilience Scale(CD-RISC)and the Mini-Mental State Exam(Mini-Mental State Exam)were used in this study.MMSE)conducted an on-site questionnaire survey among 1,427 elderly people in extreme poverty.The study analyzed 798 people who completed a cognitive function rating scale.Data were double-entered using Epi Data3.1 software,and descriptive statistical analysis,Pearson partial correlation analysis,univariate and multivariate linear regression analysis were performed using SPSS16.0.Results :(1)Most of the respondents were male(89.10%),nearly half were 70~79 years old(47.37%),most were illiterate(70.43%),and nearly 80% were unmarried(83.63%).(2)Half(52.39%)lived in a single room,and the vast majority(89.20%)received tertiary care.(3)The top three chronic disease prevalence rates were hypertension(47.74%),stroke(17.71%)and heart disease(16.60%).About 40% of the respondents(40.45%)lived completely by themselves,and nearly 20%(22.95%)of the respondents were at high risk of psychological disorders.(4)Mental resilience of the respondents: the items of mental resilience were all scored(1.44±0.53),and the items of optimism had the highest average score of(1.58±0.71).(5)Cognitive function: MMSE score was(21.00±5.47),and nearly half of the subjects had cognitive dysfunction(mild to moderate group: 15.41%;Severe group: 46.24%).(6)The results of multivariate linear regression analysis showed that: with the increase of psychological resilience(B=0.047,95%CI: 0.017~0.077),resilience(B=0.086,95%CI: 0.025~0.146),self-reliance(B=0.118,95%CI:0.037~0.200)and optimism(B=0.196,95%CI: 0.057~0.336),the cognitive function scores of the respondents increased.(7)Multivariate Pearson correlation analysis showed that the toughness dimension was positively correlated with directivity dimension and memory dimension(r=0.125,r=0.092,P <0.05);Self-improvement dimension was positively correlated with orientation dimension and memory dimension(r=0.175,r=0.078,P < 0.05).Optimism dimension was positively correlated with orientation force dimension(r=0.181,P < 0.001).Conclusions and recommendations:Conclusion :(1)The resilience,tenacity,self-reliance and optimism of the elderly in extreme poverty in rural Welfare Institutes should be improved;(2)In rural Welfare Institutes,the cognitive function of the elderly in extreme poverty is greatly or slightly impaired in every aspect,and the cognitive function is seriously impaired.(3)There was a positive correlation between resilience and cognitive function of the elderly in extreme poverty in rural Welfare Institutes.Suggestions:(1)Pay attention to the cognitive function of the elderly in extreme poverty in rural Welfare Institutes,and carry out cognitive dysfunction screening for them.(2)Implement evidence-based mental resilience training programs to intervene in the impoverished elderly in rural areas.(3)Implement intervention services by means of government purchase of services.(4)Giving full play to the supporting role of pension institutions in the intervention of cognitive function.
