Objective This study was to describe the prevalence of hypertension, overweight and obesity, and to explore the association between BMI,waist circumstance and blood pressure. The aim of understanding the association of overweight, obesity and abdominal obesity with hypertension was to provide relevant theoretical basis and scientific basis of preventing these diseases. Methods A multistage-stratified cluster sampling process was adopted according to the economic status and geographic distribution in Anhui Province to make sure the representativeness. In the first stage, five cities were randomly selected(i.e.,Suzhou,Bengbu,Maanshan,Tongling and Chizhou)and one or two country(ies)/region(s)was or were selected from these five cities. In the second stage, several villages were randomly selected from each county/region and all eligible individuals those who aged60 years old or over were included in our study. The demographic characteristics(including: sex, age, educational level, lifestyle and so on)anthropometry data(including: height, weight, waist circumstance and blood pressure)and health status were included in our study. The c2 test,t test and analysis of variance were used in the univariate analysis.Bonferroni test was used to identify the difference of two means. The logistic regression and multiple linear regression analysis were used looking for potential factors of hypertension. Results The number of male older adults was 1861(44.0%),and the number of female older adults was 2372(56.0%).The sex ratio(male to female)was 1: 1.27.The mean(SD,Standard Deviation)of height, weight and waist circumstance was 157.23(8.56)cm,57.24(10.39)kg and 84.73(12.35)cm,respectively.The mean BMI(SD)was 23.11(3.74)kg/m2.The mean value of systolic blood pressure was 133.20(20.17)mm Hg and the mean value of diastolic blood pressure was 80.12(11.32)mm Hg.The prevalence of hypertension and prehypertension in rural older adults was 57.0% and14.6%,respectively.The prevalence of overweight and obesity was 27.7% and 9.6%.The prevalence of abdominal obesity in male was lower than female(57.3% vs.63.0%,P<0.01).Results of logistic regression reflected that normal BMI/abdominal obesity, unusual BMI/ideal waist, unusual BMI/abdominal obesity were risk factors of hypertension when adjusted for sex,age category, marital status, smoking status and drinking status and the values of ORs(95%CI)were 1.40(1.20-1.64),2.06(1.44-2.95),2.60(2.23-3.03).The result of multiple linear regression analysis reflected that continued BMI and waist circumstance associated with mean arterial pressure after adjusted age, sex, educational level, marital status, drinking status, and smoking status. The standard partial regression coefficient of BMI and waist circumstance was 0.243 and 0.205,respectively.Conclusion The prevalence of overweight, obesity was still at high level and the prevalence of hypertension was increased rapidly. Thus,to assess the influence of overweight, obesity, abdominal obesity on hypertension has significant public health meaning. The results of our study reflect that the effect of BMI with mean arterial pressure was bigger than waist circumstance. Thus,to pay more attention on BMI was still the most effective measure of preventing hypertension in rural older adults.
