ObjectiveTo explore the safety and clinical value of Da Vinci robotic laparoendoscopic single-site in the treatment of early endometrial cancer by comparing the clinical data of Da Vinci robotic laparoendoscopic single-site and multi port robotic in the treatment of early endometrial cancer.MethodsClinical data of 51 patients with early endometrial cancer admitted to The First Affiliated Hospital of Nanchang University hospital from June 2019 to June 2020 were retrospectively analyzed.Patients who underwent Da Vinci robot single site laparoscopic surgery were classified into robotic laparoendoscopic single-site(n=23),and patients who underwent Da Vinci robot multi port laparoscopic surgery were classified into multi port robotic(n=28).The outcomes of both groups were analyzed and compared, including the baseline characteristics, intraoperative blood loss, operation time, lymph node dissection, conversion rate to laparotomy, intraoperative blood transfusion, intraoperative organ injury, postoperative exhaust time, visual analogue scale, postoperative complications, hospital stay and subjective evaluation of incision. To provide the basis for its clinical application by investigating the safety and clinical value of Da Vinci robotic laparoendoscopic single-site in the treatment of early endometrial cancer.ResultsBoth groups underwent successful operations without intraoperative or postoperative complications.Neither of them was transferred to ordinary laparoscopy or laparotomy.Neither group had blood transfusion during operation. The operation time of Da Vinci robotic laparoendoscopic single-site group was longer than that of robot multi port group [(178.09 ± 20.51)min vs(131.93 ± 8.89)min].The postoperative exhaust time of Da Vinci robotic laparoendoscopic single-site group was shorter than that of robot multi port group [(21.59 ± 1.15)h vs(33.09 ± 2.25)h],the visual analogue scale was lower than that of robot multi port group [3.17 ±0.58 vs 7.14 ± 1.08],and the length of total hospital stay was lower than that of robot multi port group [(4.43 ± 0.59)d vs(9.93 ± 1.51)d].The very satisfactory rates were respectively 73.91%(17 / 23)and 42.86%(12 / 28).The differences were statistically significant(P < 0.05).There was no significant difference in the amount of bleeding and the number of lymph nodes removed between the two groups(P >0.05).Conclusions1.Da Vinci robotic laparoendoscopic single-site is safe and feasible in the treatment of early endometrial cancer.2.Compared with robot multi port group,Da Vinci robotic laparoendoscopic single-site has the advantages of shorter postoperative anal exhaust time, shorter hospital stay, lower visual analogue scale, cosmetic appeal of abdominal incisions and higher postoperative patient satisfaction.3.The operation time of Da Vinci robotic laparoendoscopic single-site is longer than that of robot multi port group. It is associated with the longest operation time however, after experiencing the turning point there will be an obvious reduction.4.Da Vinci robotic laparoendoscopic single-site is an innovative way,but further prospective research with larger samples is expected. |
