Comparative Study Between Umbilical Laparoendoscopic Single-site Surgery And Conventional Laparoendoscopic Dismembered Pyeloplasty

Objective:To compare study between umbilical laparoendoscopic single-site surgery and conventional laparoendoscopic dismembered pyeloplasty,preliminary discussion the efficiency and safe of Transumbilical laparoendoscopic single-site surgery.Methods:From October 2016 to February 2018,Selected all the patients who met the inclusion criteria and confirmed to be ureteropelvic junction obstruction of The Frist People’s Hospital Of Yunnan Province.Randomly divided into single-site laparoendoscopic group and standard laparoscopic group.The standard group is divided into two ways: abdominal and retroperitoneal The various efficacy indicators between two groups were compared.Results:20 cases of single-site group and 21 cases of standard group fit for the stduy were collected.All the operations were completed successful.One patient in the single-site group had an additional operation hole and there wasn’t transit in the standard group.Three patients in the standard group had analgesics.As to drain removal time,postoperative bowl function recovery time,Intraoperative blood loss,and postoperative hospital stay,the two groups had no signifieant difference(P >0.05).The single-site group not only compared with the transabdominal standard group(161.15±36.78 vs.133.20±17.40,P=0.032)but compared with the retroperitoneal standard group(161.15±36.78 vs.128.36±24.23,P=0.013).),The operation time was statistically longer(P<0.05).The single-site group in the cosmetic effect was superior to either the abdominal standard group or the retroperitoneal standard group(P<0.001).Two groups of patients were followed up for 1-12 months,12 patients were lost to Double-J tube after they were removed,median follow-up 9.03± 3.21 months.Follow-up content for the Ultrasound and Intraudio Videoenous Urography.29 patients during follow-up had significantly improved with preoperative hydronephrosis or hydrosalpinx.No complications such as infection,urinary leakage,and bleeding occurred.Conclusions:Umbilical laparoendoscopic single-site dismembered pyeloplasty is safe and effective.On the basis of not increasing the complications,recovering fairly and guaranteeing the success rate,has the unparalleled beauty advantages of standard laparoscopic surgery,and patients with postoperative pain tolerance lighter.Single-site group through a certain technical improvement,the operation time improved significantly.This group of patients with fewer cases,the lack of large-scale long-term efficacy and follow-up,and the difficulty of surgery,learning curve longer,With the accumulation of surgical experience and improved surgical instruments,Laparoendoscopic Single-site Surgery were be better development.
