| Objective:To investigate the psychological resilience,social support and posttraumatic growth level of breast cancer patients,and to explore the relationship between PTG and psychological resilience,social support,and the changes of posttraumatic growth of breast cancer patients with different molecular types.Methods:This study used questionnaire survey research,236 breast cancer patients hospitalized in the First Affiliated Hospital of Bengbu Medical College were selected as the research objects.Convenience sampling method was used to conduct anonymous questionnaire based on social support,psychological resilience and posttraumatic growth theory.Application of Excel in data entry,SPSS 22.0 and Amos 21.0 were used to construct the structural equation.The patients with complete immunohistochemical data were divided into four groups according to molecular typing:luminal A group,luminal B group and human epidermal growth factor receptor 2(HER2)positive group and triple negative group.PTGI was used to investigate the four groups of breast cancer patients at the time of diagnosis,one month after diagnosis,three months after diagnosis and six months after diagnosis,and the differences between the groups were compared.Results:1.The average PTG score of breast cancer patients is 51.73±9.02,the average score of social support of breast cancer patients is 38.25±3.73,and the average score of psychological resilience is 55.17±10.61,which is in the middle level.PTG was positively correlated with various dimensions of psychological resilience,social support,educational level,per capita income,and religious beliefs(P<0.05);it was negatively correlated with age and lymph node metastasis(P<0.05).2.The modified structural equation model of social support,resilience and posttraumatic growth of breast cancer patients showed that the chi square value was116.859(P<0.005),the degree of freedom was 41,and the x~2/df was 2.625.The total score of PTG was positively correlated with the total score of psychological resilience(0.767,0.152,P<0.005),and positively correlated with the total score of social support(0.767,0.062,P<0.005).The level of social support could directly predict the level of PTG and resilience(P<0.005),and indirectly predict the level of PTG through resilience(P<0.005).3.The different molecular classification of breast cancer patients with puncture confirmed when PTG scores compared with no statistical significance(P>0.05);Luminal B and HER2 positive two groups of patients with breast cancer diagnosis of breast cancer for six months PTG compare the diagnosis of total score difference was statistically significant(P<0.01);Luminal A contrast Luminal B two groups of patients in the diagnosis of breast cancer after June PTG difference was statistically significant(P<0.01);Three negative HER2 positive type contrast two groups of patients in the diagnosis of breast cancer after June PTG difference was statistically significant(P<0.05).Conclusion:1.The PTG,psychological resilience and social support of breast cancer patients are at low to medium level;PTG is closely related to mental toughness and social support.Improving the level of psychological resilience and social support will help improve the level of PTG.2.Social support and resilience positively predict PTG level.Social support is positively correlated with resilience.Resilience plays a mediating role between social support and PTG.Increasing social support and resilience can increase PTG level of breast cancer patients.3.Different molecular types of breast cancer have different growth levels after trauma.Among them,patients with Luminal B and HER2-positive breast cancer have a significant increase in PTG in the short term.Clinical nursing practice should focus on helping patients with Luminal A and triple-negative breast cancer to increase PTG. |
