| Obejective:In order to provide reference for the treatment of chronic nephritis(CKD1-2)by Protecting Kidney Oral Liquid combined with conventional western medicine,the mechanism of action was explored by observing the clinical effect of Protecting Kidney Oral Liquid in the treatment of chronic glomerulonephritis of dampness-heat type with deficiency of Qi and Yin and its effect on VEGF and sFlt-1.Methods:64 patients with chronic glomerulonephritis of dampness-heat type with deficiency of Qi and Yin(Stage CKD1-2)who met the inclusion criteria of this project were randomly divided into control group(n=32)and experimental group(n=32).The control group was given routine western medicine treatment,and the experimental group was given Protecting Kidney Oral Liquid on the basis of the control group,and the treatment course was 8 weeks.The TCM syndrome score,serum vascular endothelial gro-wth factor(VEGF),serum soluble vascular endothelial growth factor receptor-1(sFlt-1),24 h urinary protein quantitation,urea nitrogen(BUN),serum creatinine(SCR),glomerular filtration rate(eGFR),serum cystatin C(CysC),total cholesterol(TC),triglyceride(TG)and Creactive protein(CRP)were observed before and after treatment.In addition,20 hea-lthy subjects were selected as the healthy control group to detect the levels of serum VEGF and sFlt-1.Result:1.Before treatment,there was no statistical significance in the general data,laboratory indicators and TCM syndrome scores between the control group and the experimental group(P>0.05),which were comparable.2.The levels of VEGF,sFlt-1 in chronic nephritis group were significantly higher than those in healthy control group,the difference of which was statistically significa nt(P<0.01).3.The effective rate of Protecting Kidney Oral Liquid combined with conventional western medicine in the treatment of chronic nephritis of dampness-heat type with deficiency of Qi and Yin was 87.1%,which was significantly higher than that of the control group(70.97%),the difference of which was statistically significant(P<0.05).4.After treatment,the TCM syndrome scores in both groups were significantly improved compared with those before treatment(P<0.01),and the improvement in the experimental group was better than that in the control group,and there was statistical difference(P<0.01).5.There were no significant difference in BUN,Scr,eGFR,CysC,TC and TG between the two groups before and after treatment(P>0.05).6.After treatment,24 h urine protein quantitation,CRP,VEGF and sFlt-1 in two groups were significantly decreased compared with those before treatment(P<0.05),and the 24 h urine protein quantitation,CRP,VEGF and sFlt-1 in the experimental group were significantly lower than those in the control group(P<0.05),the difference of which was statistically significant.7.Before treatment,sFlt-1 was positively correlated with 24 h urine protein quanttation and CRP,but not with VEGF and eGFR;There was no correlation between VEGF and 24 h urine protein quantitation,sFlt-1,eGFR and CRP.Conclusion:1.Protecting Kidney Oral Liquid combined with conventional western medicine therapyhas a significant effect on chronic nephritis of dampness-heat type with deficiency of Qi and Yin.Compared with single conventional western medicine,it has a better effect in improving clinical symptoms and reducing urinary protein.2.Protecting Kidney Oral Liquid may protect the kidney by reducing levels of VEGF and sFlt-1 in the body,relieving endothelial cell damage,decreasing urine protein,3.The generation of proteinuria maybe is related to the abnormal increase of VEGF and sFlt-1,which may reflect the damage of glomerular endothelial cells and microinflammation in the early stage of chronic nephritis to a certain extent.4.It has high safety and can be popularized in clinic as there is no obvious side effect of Protecting Kidney Oral Liquid combined with conventional western medicine therapy in the short term of this trial. |
