Research On Elderly Population Health Management Path In Community

With the deepening of the population aging in China,the health management needs of the elderly are becoming more and more urgent.The community is the basic unit of social management and an important port of health management.The health management of the elderly in the community can not only prevent and control diseases,but also is regarded as an important means to improve the health of the elderly.Therefore,it is urgent to optimize the current health management model of the elderly population,starting with strengthening the supply capacity of health management services,make resource allocation more reasonable,establish a comprehensive and continuous health management service system in community,promoting diseases prevention and rehabilitation,so as to ensure the health of the elderly population.First of all,based on the literature research method and policy analysis method,This paper analyzes the structure and implementation of the health management of the elderly population in the community from the perspective of social security,and puts forward the main problems of elderly population health management in the community.Secondly,the paper uses the questionnaire survey to investigate health condition of 320 old people in Shanghai,and collected the first-hand data,then using the structural equation model,the paper analyses the community elders’ health factors and influence ways,and build the elderly population health management framework center on health,and evaluates the supply and demand matching status of elderly population health management and management effect in the community.Then,based on the successful experience of the United States,Japan and Canada,the paper proposes the optimization idea of elderly health population management in the community,and reconstructs the elderly population health management system based on the health management theory,resource allocation theory and mechanism design theory,and proposes the implementation path.Finally,based on improving the health management performance of the elderly population in the community,the paper puts forward specific countermeasures and suggestions.The paper draws the following main conclusions: First,community elders’ health condition is closely related to health literacy,psychological health,living environment and medical environment in the community,that is to say,to create a healthy social living environment for the elderly,to build a health-centered medical model,and to help the elderly improve their self-health management ability is conducive to promoting the health level of the elderly population in the community.Second,giving full play to the macro-leading role of the government to improve the top-level design,and promote the establishment of unified health values in the society and various sectors,which is conducive to mobilize social resources and market enthusiasm,so as to improve the community health management service system and meets the multi-level health needs of the elderly.Thirdly,the family doctor system can effectively promote the accurate development of the elderly population health management in the community,greatly improve the accessibility,comprehensiveness and continuity of health services for the elderly population,so as to move forward the disease prevention threshold,promote the rational allocation of health resources,and improve the health level of the community elderly.
