| ObjectiveThrough the use of Perineal Assessment Tool(Perineal Assessment Tool;PAT)screened out high-risk groups of ICU elderly patients with incontinence-related dermatitis(IAD),and conducted predictive nursing intervention.To explore the clinical application effect of oral skin care powder combined with skin protective film and traditional Chinese medicine lianqinqinre friction in the prevention and treatment of IAD in elderly ICU patients,and to make a comparative analysis,so as to provide a reference for the formulation of clinical predictive nursing plan of IAD in elderly ICU patients and the specific treatment measures of IAD.MethodsChoose between January 1,2018 and December 31,2018,in a first-class ternary hospitals in Shandong province elderly patients with comprehensive ICU,into the inclusion standard 100 patients as research samples,through the use of perineum risk assessment tool to screen out with IAD of high-risk groups86 cases.Using random number table method,the patients were divided into observation group and control group,with 43 cases in each group.the two groups of patients were given routine care first,namely using 37℃-39℃ warm water after each stool flushing perineal skin,especially the skin folds were cleaned.After cleaning,the perineum residual water was sucked dry with a clean and soft towel,and the movements were gentle.Timely change the pad and bedsheet,keep the bedsheet clean and dry.Patients were given 1 turn over every 2 hours.Patients in the control group were given the same routine care as those in the observation group.Patients in the control group were given oral care powder combined with skin protective film for nursing intervention.Patients in the observation group were given traditional Chinese medicine lianqinqinre friction nursing intervention.Compared two groups of patients in the incidence of IAD,time,severity,nursing efficiency,healing time,nursing time of the operation.Results1.Comparison of general clinical data:A total of 100 elderly patients admitted to the ICU were screened for skin risks using PAT,and 86.00%(86 cases)of them were at risk of IAD.Including 50 males and 36 females,aged 65-82 years old.Using a random number table method,divide into two groups and there was no significant difference in age,gender,PAT score,incontinence type and incontinence frequency between the two groups(P>0.05).2.Comparison of IAD incidence in the two groups:the incidence of IAD in the control group was 37 cases,the incidence rate was 86.05%.The number of IAD cases in the observation group was 28 cases,the incidence rate was 65.12%.The difference between the two groups was statistically significant(P<0.05)。3.Occurrence time of IAD in the two groups:the occurrence time of IAD in the control group was(4.87± 1.02)days,while that in the observation group was(6.24± 1.57)days,and the difference between the two groups was statistically significant(P<0.05).4.Severity of IAD in the two groups:in the control group,there were 21 mild cases,11moderate cases and 5 severe cases of IAD,while in the observation group,there were 19 mild cases,8 moderate cases and 1 severe case.The difference between the two groups was statistically significant(P<0.05).5.Comparison of IAD nursing efficiency between the two groups:after different intervention measures were taken in the patients with IAD in the two groups,19 patients in the control group were cured,13were effective,and the effective rate was 86.48%;while 21 patients in the observation group were cured,6 were effective,and the effective rate was 96.43%.The difference between the two groups was statistically significant(P<0.05).6.IAD healing time in both groups:The healing time was(5.51±1.02)days in the control group and(4.32±0.78)days in the observation group.There was a significant difference between the two groups(P<0.05).7.The average nursing operation time of the two groups:that is,the average nursing intervention time of each patient,the nursing operation time of the control group was(8±0.82)minutes,and that of the observation group was(3±0.18)minutes,the difference between the two groups was statistically significant(P<0.001).Conclusions1.On the basis of using PAT to screen out the population at high risk of IAD,traditional Chinese medicine lianqinqinre friction in elderly patients in the ICU IAD prevention effect is better than that of skin care powder joint skin protective film,its low incidence,severity,delaying the occurrence time,nursing operation is simple,save the time of care and can effectively reduce the nursing workload and improve the quality of nursing service.2.In the treatment of IAD in ICU elderly patients,the effect of traditional Chinese medicine lianqinqinre friction is better than that of stoma skin care powder combined with skin protective film,with high cure rate and effective rate and short healing time,which can shorten the hospitalization time of patients,improve the satisfaction of patients and their families,and save medical resources. |
