Objective:To explore the effect of the application of 4R crisis management theory in skin management for fecal incontinence in patients with ICU.Methods:Totals of 75 cases of ICU fecal incontinence patients from January2016 to December 2016 in our hospital,who received conventional fecal incontinence skin care method were set to be the control group(group C).The other 75 cases of ICU fecal incontinence patients from January 2017 to December 2017 in our hospital,who received the 4R crisis management theory in skin care were set to be the management group(group G).The skin care of the patient in group G by 4R crisis management theory from the four stages of reduction,preparation,response,and recovery.The incidence of IAD,the time of IAD,the severity of IAD,and the healing of skin lesions after IAD were observed in both groups.Results:Incontinence-associated dermatitis occurred in 18 patients in the control group and the incidence was 24%,the incidence of IAD was significantly higher than that in the management group(6%)(P<0.05).The average time of IAD occurred in the control group(2.48±0.95)was significantly earlier than that of the management group(3.89±1.01)(P<0.05).The IAD severity score in the control group(23.40±4.12)was significantly higher than that in the management group(12.80±3.45).The severity of IAD in the control group was more severe than that in the management group(P<0.05).In the management group,5 IAD patients were cured,1 was improved,and the effective rate was 100%.The curative effect was better than that of the control group(50%)(P<0.05).Conclusion:4R crisis management can effectively improve the skin care quality of nurses for ICU fecal incontinence patients,reduce the incidence of IAD events,promote the healing of skin lesions after IAD occurs,ensure the patient's skin care predictability,continuity,and systemic.
