Obective:To evaluate the clinical efficacy of natural orifice specimen extraction surgery of colorectal cancer and sigmoid cancer, and to explore the feasibility of this operation in the operation of colorectal cancer,as well as its advantages and disadvantages in clinical application. To provide ideas for the further promotion and development of the natural orifice specimen extraction surgery. Methods:Retrospectively collected first affiliated hospital of kunming medicaluniversity in oncology department in June 2016 to November 2020 and accept the rectum and sigmoid colon cancer diagnosed by radical surgery for the clinical data of 142 patients, preoperative staging exclude T4 tumors, ultimately 83 patients were included in the study, which according to the specimen out way is divided into two groups:The Laparoscopic combined Noses(Noses)and the Conventional Laparoscopic(LAP).34 in the Nose group and 49 in the LAP group. The general information, operative time, intraoperative blood loss, number of lymph nodes detected after surgery, postoperative defecation time, postoperative additional analgesics, postoperative hospital stay, postoperative serious complications, incision infection and incision satisfaction were observed and compared between the two groups. Results:A total of 83 patients were included in the study, including 34 in the Nosesgroup and 49 in the traditional laparoscopic group. There were no significant differences between the two groups in general information, operative time, intraoperative blood loss, the number of lymph nodes detected and postoperative complications(P>0.05).In terms of postoperative defecation time, postoperative hospitalization days, additional analgesics, incision satisfaction, postoperative combined incision infection rate, the NOSES in the Nose group were significantly better than those in the LAP group(P<0.05).Conclusions:1.NOSES technology in operation time, intraoperative blood loss, detection of lymph node number, and severe postoperative complications in no obvious difference with traditional specimens removed method, preliminary confirmed that the nonlocal terminal(T3)or less rectum and sigmoid colon carcinoma with NOSES technique in laparoscopic surgery safety,to gradually develop in the future clinical work.2.In laparoscopic surgery for non-locally advanced(≤T3)rectal cancer and sigmoid cancer, the use of the Nosing technique has advantages in alleviating postoperative pain, shortening the length of hospital stay, accelerating the recovery of gastrointestinal function, and increasing patient satisfaction with the incision.
