Objective: To comparative analyze the short-term efficacy of radical resection of colorectal cancer and traditional laparoscopic radical resection of colorectal cancer by natural lumen specimen(NOSES IV type)to explore the optimal surgical treatment for mid-high rectal and sigmoid colon.Methods: 38 patients with moderate to high rectal cancer and sigmoid colon cancer admitted to the Department of Gastrointestinal Surgery,Second Affiliated Hospital,from April 2017 to May 2018 were enrolled transrectally(observation group);40patients underwent conventional endoscopic surgery during the same period Comparable to the control group,comparative analysis of relevant clinical data.Results: In the study group,2 cases were transferred to open abdomen,and 1 cases in the control group were transferred to open abdomen.The amount of intraoperative bleeding,the length of abdominal incision,the first time of anus exhaust,the time of hospitalization,infection rate and quality of life and post-anal bowel function of the study group were superior to those of the control group.The difference was statistically significant(P < 0.05),and the number of lymph dissection,the distance of the distant margin of the tumor,the resection of the tumor in the two groups.There was no significant difference in size between the two groups(P > 0.05).The short-term immune recovery in the two groups was statistically significant(P < 0.05),and the observation group was better than the control group.Conclusion: NOSES IV are safe,feasible and effective in the middle and high rectum and rectal cancer root types,and their short-term efficacy is superior to traditional endoscopic colorectal cancer surgery.
