Objectives:Analyze the clinical data of lung cancer patients with pneumonectomy,explore the risk factors of early postoperative complications,and provide help for the prognosis of early postoperative complications.Methods:The clinical data of lung cancer patients undergoing pneumonectomy in the First Department of Thoracic Surgery and the Second Department of Thoracic Surgery of the Third Affiliated Hospital of Kunming Medical University from January 2013 to January 2021 were retrospectively analyzed,and the preoperative and intraoperative data of the patients were collected.For the occurrence of early postoperative complications,Logistic regression was used to analyze the risk factors of early complications after pneumonectomy for lung cancer patients.Results:Enrolled 113 cases of lung cancer patients with pneumonectomy,75 males and 38 females,aged 37-67 years,the incidence of early postoperative complications was 46%(53/113),of which 29 were cardiovascular complications.There were 17 cases of respiratory system complications and 7 cases of other system complications.Single factors indicating early risk factors postoperative complications were:preoperative underlying disease,FEV1/FVC%<70,FEV1<2L,intraoperative partial pericardectomy,pleural adhesions,operation time>3h,intraoperative blood transfusion>1000ml,Total blood transfusion>1400ml,total drainage>1000ml,postoperative hospital stay>20d.Multivariate analysis shows that there were underlying diseases before operation,FEV1<2L,intraoperative partial pericardectomy,pleural adhesions,operation time>3h,total blood transfusion>1400ml,total drainage>1000ml,postoperative hospitalization days>20d.early risk factors complications.Multivariate analysis of subgroups showed that FEV1<2L and postoperative hospital stay>20d are early risk factors postoperative respiratory complications,preoperative underlying diseases,partial pericardectomy,intraoperative lens fluid>500ml,FEV1<2L,operation time>3h is an early risk factor postoperative cardiovascular system complications;operation time>3h,intraoperative fluid infusion>1500ml,total blood transfusion>1400ml,postoperative hospital stay>20d are other early postoperative complications Risk factors.Conclusion:Poor preoperative lung function and enduring postoperative hospital stays increase the risk of early respiratory complications in patients with lung cancer after pneumonectomy,while preoperative lung function is poor,partial pericardectomy,intraoperative fluid infusion,and postoperative hospital stays The growth increases the danger of early postoperative complications of the cardiovascular system Skilled and delicate operations,strict hemostasis,intraoperative crystal fluid intake limitation,control of infusion per unit time,and shortened operation time are helpful to reduce the incidence of early complications after lung resection in patients with lung cancer.
