Objective:To investigate the effect of pneumonectomy in comparison with video-assisted thoracoscopy and thoracotomy in patients with non-small cell lung cancer,and to analyze the influence of the two surgical methods on postoperative pneumonectomy in patients with video-assisted thoracoscopy and thoracotomy,so as to provide reference for clinical guidance of perioperative treatment.Methods:A total of 58 patients who underwent thoracoscopic and open-thoracotomy pneumonectomy in the Department of Thoracic Surgery of the Affiliated Hospital of Zunyi Medical University from January 2017 to December 2020 were collected by retrospective analysis.According to the inclusion criteria,38 patients who met the criteria were finally selected,among which 17 patients underwent video-assisted thoracoscopy pneumonectomy.Thoracotomy pneumonectomy was performed in 21 cases.Perioperative clinical indexes,postoperative complications,hospitalization period and treatment costs of 2 groups were observed.Results SPSS 18.0 statistical software was used for analysis.Measurement data were expressed as mean standard deviation(x ±s),and normal test was performed.Data in line with normal distribution were tested by independent sample t test,while those in discord were tested by rank sum test.Results: All the 38 patients who met the criteria were selected,including 17 patients underwent video-assisted thoracoscopy pneumonectomy and 21 patients underwent Thoracotomy pneumonectomy.There were no significant differences in operative time and incidence of postoperative complications between the two groups(P >0.05).In the thoracoscopic group,intraoperative blood loss,postoperative catheterization time,postoperative total drainage volume,postoperative dose of dezocine and demerol,postoperative length of hospital stay and hospitalization cost were(63.68±24.32)ml,(8.00±4.18)days,(1495.26±532.63)ml,(7.06±19.93)mg,(4)mg,respectively.41±3.48 mg,(12.37±4.84)days and(67218.68±15884.43)yuan,They were significantly lower than those in the thoracotomy group(157.63±64.71)ml,(10.89±2.28)days,(2236.32±824.36)ml,(34.29±55.55)mg,(8.10±5.36)mg,(15.89±4.23)and(81402.53±15059.77)days)and there was statistical significance between the two groups(P <0.05).Conclusion: Through retrospective analysis,we believe that the video-assisted thoracoscopy pneumonectomy group has some advantages over the open thoracotomy group.The advantages of thoracoscopic pneumonectomy were less intraoperative blood loss,less postoperative drainage,earlier extubation time of the drainage tube,less pain on postoperative and reduced hospital stay and hospitalization cost compared with the thoracotomy group.However,there were no statistical differences between the thoracoscopic group and the thoracotomy group in terms of operative time and incidence of postoperative complications.In this study,it was considered that thoracoscopic pneumonectomy had similar safety and certain advantages compared with thoracotomy.
