[Objectives] To investigate the related factors and prevention strategies of early postoperative complications in patients with left lung cancers after pneumonectomy.[Methods] Clinical data of postoperative complications within 30 days after pneumonectomy in 95 cases of left lung cancer from May 2002 to February 2016 in our hospital were retrospectively analyzed.12 patients’ indicators including their age, sex, smoking, preoperative lung function, chronic obstructive pulmonary disease, history of cardiovascular disease, history of diabetes, surgery time, surgical approach, surgical blood loss, duration of mechanical ventilation, wound pain and type of postoperative complications were collected and analyzed with a chi-square test to identify risk factors associated with postoperative complications.[Results] Postoperative arrhythmia occurred in 25 cases (26.32%), pulmonary infection in 14 cases (14.74%), active bleeding in 2 cases (2.1%) and bronchial pleural fistula in 1 case (1.1%) of 95 patients. Exacerbation of pulmonary infection in 2 cases resulted in circulatory failure and caused death. The patient with pleural fistula discharged and was lost to follow-up. The remaining patients improved after active treatment and then discharged.[Conclusions] 1. Early postoperative complications in left pneumonectomy include cardiac arrhythmia, pulmonary infection, hemothorax and bronchial fistula, in which the most common was cardiac arrhythmia followed by pulmonary infection.2. Elderly, pulmonary dysfunction, pericardial pneumonectomy, chronic obstructive lung disease, a history of cardiovascular disease, excessive blood loss, long mechanical ventilation, incision pain≥Ⅱ are risk factors associated with postoperative arrhythmia.3. Elderly, smoking, pulmonary dysfunction, chronic obstructive lung disease, diabetes, long surgery time, long mechanical ventilation are risk factors associated with postoperative arrhythmias.
