Study On The Relationship Between Congenital Five Evolutive Phases And Six Climatic Factors And Gout

Objective:To study the correlation between the incidence of gout and congenital five evolutive phases and six climatic factors,we can infer the law of its incidence,hoping to provide a new idea for the etiological study of gout and the exploration of the opportunity of preventive treatment from the aspect of traditional Chinese medicine.Methods:A retrospective case-control study was conducted.The gout patients who visited the rheumatology department of our hospital from February 4,2020 to February 4,2021 were selected as the case group.The control group was the same period in our hospital physical examination center physical examination and then screened out the healthy population.The subjects’ gender,age,date of birth,date of treatment and exposure history of gout related risk factors were collected,and the relevant contents were sorted out with the record form(see Appendix 1 and Appendix 2),and then the five evolutive phases and six climatic factors was calculated.Then,spss22.0 was used for data processing.First,the five evolutive phases and six climatic factors and risk factors were statistically analyzed.Then,the significant five evolutive phases and six climatic factors were analyzed by univariate factor logistic regression analysis.Finally,the five evolutive phases and six climatic factors and risk factors with P<0.15 were analyzed by multivariate logistic regression analysis to explore the correlation and correlation degree with the occurrence of gout.Results:1.General informationFrom February 4,2020 to February 4,2021,a total of 424 subjects were included in this study.A total of 212 cases were included in the case group,including 193 males and 19 females.The control group included 212 healthy people,including 155 males and 57 females.The age quartiles of the two groups were 32.25,40.5,50 and 31,40,49 respectively.The average age rank of the case group was 219.01,while that of the control group was 205.99.The results showed that the age distribution was not statistically significant,but the gender distribution was on the contrary,which was correlated with gout.2.The five evolutive phases and six climatic factors and risk factorsDescriptive statistics of the case group population:the number of people who were born in Ren-years was the largest(17.45%),Gui-years was the second(15.09%);Xu-years was the largest(10.38%),Shen-years and Hai-years were the second(9.91%);SiTian ZaiQuan in the year when they were born was mostly in the greatwater-TaiYang and great earth-TaiYin(19.34%),and at least in the great gold-Yangming and little fire-Shaoyin,accounting for 14.62%;the number of people whose main Qi was great earth-TaiYin at birth was the most,accounting for 20.75%;the number of people whose ZhuQi was the little fire-Shaoyang fire was the least,accounting for 12.26%;the number of people whose KeQi was great earth-TaiYin at birth was the most,accounting for 20.75%.The number of people whose KeQi was the little fire-Shaoyang fire was the second,accounting for 17.92%.The number of women who were pregnant in the year of Ren-years was the largest,followed by those in the year of Xin-years,accounting for 17.45%and 16.03%respectively.The number of the Jia-years of pregnancy was the least,accounting for 6.6%;the number of people whose DiZhi were Wei-years and Hai-years was the same and the most,accounting for 10.85%,while the number of people whose DiZhi were Shen-years was the least,accounting for 5.56%;At the time of pregnancy,the most of SiTian ZaiQuan was great earth-TaiYin and great water-TaiYang(19.34%),and the least of SiTianZaiQuan was the little fire-Shaoyang fire and little wood-JueYin(13.21%);during pregnancy,the number of people whose ZhuQi was great water-TaiYang was the most,accounting for 22.64%,while the number of people whose ZhuQi was great gold-Yangming was the least,accounting for 13.68%;during pregnancy,the number of people whose KeQi was the little fire-Shaoyang fire was the most(19.34%).Test analysis:the analysis of five evolutive phases and six climatic factors and risk factors showed that the P values of tiangan at birth and gestation were 0.004 and 0.013 respectively,which were all<0.05,with statistical significance;the P values of other luck factors were all>0.05,which showed that DiZhi,SiTian ZaiQuan,ZhuQi,KeQi at birth,and Dizhi,Sitian Zaiquan ZhuQi,KeQi at gestation had no statistical significance Family genetic history,drinking habits and high purine diet were positively correlated with gout(P<0.05).The P value of strenuous exercise was 0.071,indicating that there was no statistical significance,but it could not indicate that strenuous exercise had no correlation with the occurrence of gout.Univariate logistic regression analysis:TianGan at birth and pregnancy were significantly correlated with the risk of gout.In the year of birth,the lowest risk of gout was in Yi-years(Shaoshang Jinyun),the highest was in Ren-years(Taijiao Muyun),which was 3.671 times of Yi-years(P=0.003),and the second was in Gui-years(Shaozheng Huoyun),which was 3.571 times of Yi-years(P=0.005).In the year of pregnancy,the lowest risk of gout was in Wu-years(Taizheng Huoyun),the highest was in Ren-years(Taijiao Muyun),which was 4.191 times higher than in Wu-years(Taizheng Huoyun)(P=0.001),the second was in Xin-years(shaoyushuiyun),which was 2.935 times higher than that.Gui-years may be a risk factor for gout(P=0.122<0.15).Multivariate logistic regression analysis:In the year of birth,the highest risk of gout was in Ren-years(Taijiao Muyun),followed by Gui-years(Shaozheng Huoyun);the risk of gout was 3.966 times in Ren-years and 3.91 times in Gui-years(Shaoshang Jinyun)(P=0.003)and 3.91 times in Gui-years(Shaoshang Jinyun)(P=0.004);Jia-years may be a risk factor for gout(P=0.132<0.15).In the year of pregnancy,the highest risk of gout was found in Ren-years,followed by Xin-years(Shaoyu Shuiyun).The risk of gout in Ren-years and Xin-years was 4.422 times(P=0.001)and 2.786 times(P=0.018)higher than that in Wu-years(Taizheng Huoyun),respectively.Family genetic history,drinking habits,high purine diet and severe exercise were independent risk factors for gout.Conclusion:1.The occurrence of gout is correlated with the occurrence of TianGan(Zhongyun)at the time of birth and pregnancy.Ren-years(Taijiao Muyun),Gui-years(huoyun)and Xin-years(Shaoyu Shuiyun)are the risk factors of gout,and ren-years(Taijiao Muyun)is the most dangerous.2.The Jia-years of birth and the Gui-years of pregnancy may be the risk factors of gout.3.Except for TianGan(Zhongyun),there is no significant difference between the occurrence of gout and other five evolutive phases and six climatic factors including Sitian Zaiquan,ZhuQi and KeQiat at pregnancy and birth.
