| Purpose:To study the mechanism of Shoutai Wan in the treatment of early threatened abortion through network pharmacology,and to clarify the effective components,targets and signal pathways of Cuscuta chinensis,Loranthus parasiticus,Dipsacus asperata and Ejiao in Shoutai Wan in the treatment of early threatened abortion,and to explore the curative effect of Shoutai Wan in the treatment of early threatened abortion through clinical observation.Method:1.Network pharmacology research: Genecards database was searched to obtain the corresponding target genes of early threatened abortion.BATMAN-TCM and TCMSP databases were used to obtain the active ingredients and target genes of each traditional Chinese medicine in Shoutai Wan.The target genes of the two drugs were analyzed by Venny to screen out the common genes of disease drug.The visualization network of Shoutai Wan early threatened abortion was constructed by Cytoscape software.With the help of David online tool,GO function analysis and KEGG signal pathway analysis of common genes were carried out.The common genes were introduced into the string database for PPI network visualization,and the results were imported into the Cytoscape software.The core genes were screened by using the plug-in MCODE.Finally,the therapeutic effect of core gene in early threatened abortion was obtained through Pub Med database.2.Clinical study: 60 patients with early threatened abortion were randomly divided into observation group and control group,30 cases in each group.The observation group was treated with progesterone and diltiazem.Spss25.0 software was used to analyze the clinical total effective rate,TCM syndrome score before and after treatment,serum h CG level,progesterone level and estradiol level of the two groups.Results:1.Network pharmacology research: a total of 175 genes were obtained by Venny analysis of the target genes of drugs and diseases,and55 key target genes such as LEP,IL-6,IL-10,m TOR,HMOX1 and PTGS2 were screened out.Go analysis showed that 676 biological processes were involved,including response to estradiol,angiogenesis,positive regulation of nitric oxide biosynthesis,positive regulation of cell proliferation and negative regulation of apoptosis.KEGG signaling pathway involves 129 pathways,including HIF-1 signaling pathway,TNF signaling pathway and prolactin signaling pathway.2.Clinical research: In the observation group,1 case fell off and 29 cases were observed;in the control group,2 cases fell off and28 cases were observed.the total effective rate of the observation group was96.6%,and that of the control group was 82.1%.After treatment,the TCM syndrome score(P < 0.05),serum h CG level,progesterone level and estradiol level of the observation group were significantly better than those of the Control group(P < 0.01).Conclusion:1.(1)According to the research results of network pharmacology,LEP,IL-6,IL-10,m TOR,HMOX1,PTGS2 and other core genes can play the role of fetal protection by regulating placental metabolism,regulating maternal innate and adaptive immunity,inhibiting uterine contraction and promoting angiogenesis and development in placental tissue;In addition,the common gene contains PGR(progesterone receptor),which indicates that Shoutai Wan can regulate the level of serum progesterone in early pregnancy(2)HIF-1 α、VEGF、EGF、EGFR and other genes are enriched in HIF-1 signaling pathway to regulate placental hypoxia tolerance,proliferation and differentiation of trophoblast,and promote angiogenesis;Tumor necrosis factor(TNF)signaling pathway and prolactin signaling pathway can regulate the intrauterine environment and facilitate embryo implantation(3)The enrichment analysis of go gene function involves the response to estradiol,which indicates that Shoutai Wan plays a certain role in the regulation of serum estrogen level in early pregnancy.2.Through clinical observation,it is further verified that Shoutai Wan has significant advantages in the total effective rate of early threatened abortion,which can improve the TCM syndrome score,serum HCG level,progesterone level and estradiol level of patients. |
