Abstract/Summary:
Objective: By searching the literature on the treatment of limb lymphedema with oral Chinese medicine,This paper summarizes the rule of Chinese medicine for the treatment of lymphedema of limbs.and observed the curative effect of Yinchen Simiao Decoction in the treatment of lower limb lymphedema(dampness heat syndrome)and its influence on fibrinogen level.Methods: The literatures about the treatment of limb lymphedema by oral administration of traditional Chinese Medicine published in CNKI,Wanfang Data,Chongqing VIP and CBM were retrieved and extracted from 1989 to 2019,and the data mining was carried out with the aid of traditional Chinese medicine inheritance platform to explore the rule of drug use.30 patients with lower limb lymphedema(syndrome of damp-heat pouring down)treated in peripheral vascular department of Affiliated Hospital of Shandong University of Traditional Chinese Medicine were selected as observation group,and 30 healthy people in the physical examination center of Affiliated Hospital of Shandong University of traditional Chinese medicine were selected as the control group.The treatment group was treated with Yinchen Simiao Decoction for 14 days.The changes of limb circumference,TCM syndrome score and plasma fibrinogen levels before and after treatment were observed.Results: In total of 160 prescriptions and 198 Chinese herbs were included in the data mining research.The top 10 Chinese herbal medicines ranked in frequency were as follows:Astragalus membranaceus,Poria cocos,Atractylodes macrocephala,Angelica sinensis,Ramulus Cinnamomi,Ligusticum chuanxiong,coix seed,Ramulus Mori,liquorice and Codonopsis pilosula.Among all the drugs used,warm drugs were the most,followed by mild and cold drugs.The five flavors are mainly sweet,bitter and pungent.There are obviously more drugs for liver,spleen and heart meridian in meridian attribution.According to the association rules,the top five commonly used drug pairs are “Astragalus-Atractylodis Macrocephala” “Astragalus-Cassia Twig” “Angelica-Poria Cocos” “Astragalus-Angelicae”and “Atractylodis Macrocephala-Poria Cocos”.There were 9 core drugs,including Astragalus,angelica,chuanxiong,Poria cocos,Polyporus,Alisma,coix seed,Sangzhi and Guizhi.And extracted 13 new prescriptions for the treatment of limb lymphedema.The results of clinical study showed that the fibrinogen level of the treatment group(4.42±0.54)g/L was significantly higher than that of the control group(2.95±0.45)g/L,the difference was statistically significant(P<0.05).Compared with before treatment,TCM syndrome score(3.56±2.14)was significantly lower than before treatment(11.70±1.95)(P<0.05),and plasma fibrinogen was significantly lower than before treatment(P<0.05).3 cases were basically cured,13 cases were markedly effective,14 cases were effective,and 0 case was ineffective.The total effective rate was 100%.Conclusion: Commonly used medicines for the internal administration of traditional Chinese medicine in the treatment of limb lymphedema include Astragalus membranaceus,Tuckahoe,Atractylodonis tractylodonis,Angelica sinensis,cassia twood,Chuanxiong rhizoma,coix seed,etc.,and the prescription of Jiantang focuses on invigorating qi and invigorating spleen,nourishing water and reducing swelling,supplemented by promoting blood circulation and removing blood stasis,clearing heat and detoxification,and warming Yang and nourishing water.Clinical studies have shown that there is a certain correlation between plasma fibrinogen level and lower extremity lymphedema(dampness heat syndrome).Yinchen Simiao decoction can improve TCM syndrome score and reduce plasma fibrinogen level in the treatment of lower extremity lymphedema.
