ObjectiveBased on the clinical records of traditional Chinese medicine(TCM)in the treatment of cardiac neurosis, data mining technology was used to analyze and summarize the law of syndrome differentiation and treatment of cardiac neurosis to provide some reference for the treatment of this disease.MethodsSearched China National Knowledge Infrastructure(CNKI),China Biology Medicine(CBM),Wanfang data knowledge service platform and Chinese Scientific Journals Full-text Database(VIP)to obtain the published medical records of traditional Chinese medicine in the treatment of cardiac neurosis.Collected the data of cardiac neurosis cases that were treated effectively with traditional Chinese medicine in the outpatient department of Guangdong Provincial Hospital of Chinese Medicine from January 2018 to May 2020.Extract the information of symptoms, tongue condition, pulse condition, disease syndrome elements and Chinese medicine from the medical records.WPS form was used to establish the corresponding database and the data was standardized.WPS Office 2016 software was used for frequency analysis,SPSS Statistics 19.0 software was used for cluster analysis and factor analysis, and SPSS Modeler 18.0 software was used for association rule analysis to explore the syndrome differentiation and medication rules of TCM treatment in cardiac neurosis.ResultsA total of 302 medical records that meet the purpose of the study were included. The analysis results show that there were 110 kinds of symptoms involved. The common symptoms were chest tightness, palpitation, insomnia, shortness of breath, irritability, chest pain, dizziness, fatigue, dreamy, anorexia, tiredness, dry mouth, sigh, bitter mouth, easy to panic, loose stool, dry stool, headache, sweating, abdominal distension, etc.Among them, chest tightness, palpitation, insomnia, shortness of breath, and irritability occurred more frequently. In the tongue condition, the color of tongue with high frequency were dark red, light red, light white, red, light dark, and the tongue coating with high frequency were thin white fur, white fur, white greasy fur, yellow greasy fur, thin yellow fur.In the pulse condition, the pulse with higher frequency were string pulse, fine pulse, smooth pulse, deep pulse. In terms of disease syndrome elements, there were a total of 11 syndrome elements, which were qi deficiency, blood deficiency, yin deficiency, yang deficiency,qi stagnation, phlegm, blood stasis, internal dampness, actual heat, actual cold, and qi inverse.Among them,qi deficiency,qi stagnation, phlegm, blood stasis appeared more frequently. In most patients with cardiac neurosis, there were two or three associated syndrome elements at the same time. The common disease syndrome elements were clustered and finally two categories with research significance were obtained. In terms of medicinal,a total of 229 kinds of Chinese medicine were used in 302 prescriptions, involving 18 categories of Chinese medicine and 39 kinds of main functions, and the total frequencies of medicinal used were 3949.Among them, the drugs with high frequency of use were supplementing medicinal,mind-calming medicinal and qi-regulating medicinal.Among the supplementing medicinal,qi-supplementing medicinal were used most frequently,at 15.98%,and among the mind-calming medicinal, the heart-nourishing and mind-calming medicinal were used most frequently,at 10.23%.Except Glycyrrhizae Radix Et Rhizoma, there were 54 Chinese medicines used frequently(frequency≥20),of which the top ten were Bupleuri Radix,Poria,Ziziphi Spinosae Semen,Paeoniae Radix Alba,Pinelliae Rhizoma,Salviae Miltiorrhizae Radix Et Rhizoma,Atractylodes Macrocephala,Angelicae Sinensis Radix,Astragali Radix, and Citri Reticulatae Pericarpium.Cluster analysis was carried out on 54 Chinese medicines used in high frequency.Combined with professional knowledge,a total of 14 categories were finally obtained. The suitability test of factor analysis for Chinese medicine with high used frequency was carried out,and the result showed that KMO value was 0.631 and the P value of Bartlett spherical test was 0.000,which indicated that it was suitable for factor analysis.A total of 18 common factors were extracted by principal component analysis.Based on the Apriori model, under the condition that the minimum support was 10%,the minimum confidence was 70%and the minimum promotion was 2%,the association rules of the medicinal with high frequency of use were analyzed, and a total of 16 strong association rules were obtained.ConclusionThe symptoms of patients with cardiac neurosis were diverse, and the common symptoms are chest tightness, palpitations, insomnia, etc. The common disease syndrome elements were qi deficiency,qi stagnation, phlegm, blood stasis, blood deficiency, and yin deficiency.Among them,qi deficiency was the most common syndrome element in deficiency syndrome, and qi stagnation, phlegm and blood stasis were the most common syndrome elements in solid syndrome. It was common to have two or more syndrome elements at the same time.Among them,qi deficiency,qi stagnation, phlegm and blood stasis mostly existed together, internal dampness and real heat mostly existed together. In the treatment of cardiac neurosis, traditional Chinese medicine mainly applied the methods of dispersing the liver and regulating qi,replenishing qi and enriching blood, resolving phlegm and dampness, activating blood circulation and removing blood stasis, nourishing the heart and calming the mind, with particular emphasis on the application of replenishing qi,calming and regulating qi.The commonly used prescriptions mainly include Guizhi Gancao Longgumuli Decoction,Gualou Xiebai Banxia Decoction,Erchen Decoction,Wendan Decoction,Tianwang Buxin pill,Xuefu Zhuyu Decoction,Chaihu Shugan Powder,Shengmai Powder,Chaihu jia Longgumuli Decoction,Xiaoyao Powder and so on.
