BACKGROUND:Lumbar disc herniation(LBH)is a common and frequently occurring disease in clinic.It is a series of symptoms and signs on the basis of degenerative disease of lumbar disc,such as imbalance of internal and external mechanical balance of spine,rupture of annulus fibrosus,protrusion of nucleus pulposus,stimulation or compression of corresponding spinal nerve root or spinal cord.Lumbar disc herniation combined with radiculopathy(lbhr)refers to a series of symptoms such as pain,weakness and numbness in the muscle or skin distribution area caused by the local location of the intervertebral disc and its contents moving beyond the edge of the normal intervertebral disc.Numbness and pain are the two main symptoms of LDH.Lbhr is often classified as "low back pain","Bi syndrome"and "stubborn numbness" in traditional Chinese medicine.At present,there are many kinds of treatment methods for lumbago,but there are few effective methods for the treatment of nerve root symptoms mainly numbness,and the clinical effect is poor.Acupotomology has a good clinical effect in the treatment of LDH.Acupotomology can release muscle and nerve at the same time,which has advantages in the treatment of ldhr.In clinical practice,we found that needle knife nerve stimulation is effective and safe in relieving LDHR.Objective to explore the clinical efficacy of acupotomy nerve stimulation in the treatment of LDHR and to provide practical basis for clinical Acupotomy nerve stimulation in the treatment of LDHR.OBJECTIVE:1.To clarify the role of acupotomy nerve stimulation in the treatment of LDH and radiculopathy through randomized controlled trials,and to evaluate its effect on improving the visual analogue scale(VAS)of lumbar pain and radiculopathy compared with electroacupuncture and acupotomotor nerve stimulation,as well as the JO A score,effective rate and incidence of adverse events,so as to guide the clinical application of acupotomy nerve stimulation in the treatment of LDHR.2.The published randomized controlled clinical trials of nerve stimulation in the treatment of LDH were searched by computer.The data of effective rate,VAS score and incidence of adverse events were extracted,and then meta-analysis was performed by Revman 5.3 software.The heterogeneity and bias risk were evaluated.METHOD:1.To clarify the role of acupotomotor nerve stimulation in the treatment of LDH and radiculopathy through randomized controlled trials,and to evaluate its effect on improving the visual analogue scale(VAS)of lumbar pain and radiculopathy compared with electroacupuncture and acupotomotor nerve stimulation,as well as the JOA score,effective rate and incidence of adverse events assessed by the Japanese Orthopaedic Association,so as to guide the clinical application of acupotomotor nerve stimulation in the treatment of LDHR Practice.2.Meta analysis was used to analyze the clinical efficacy of LDH conservative therapy,so as to provide high-level evidence for the safety and effectiveness of nerve stimulation.RESULT:1.After treatment,the VAS scores of the two groups were significantly improved compared with those before treatment(P<0.05).The improvement of the Acupotomy group was better than that of the conventional Acupotomy group and the electroacupuncture group(P<0.05).2.By JOA score scale,the dysfunction of the two groups were significantly improved after treatment(P<0.05).The Acupotomy group was significantly better than the conventional Acupotomy group and electroacupuncture group(P<0.05).3.Previous studies have shown that nerve stimulation has good clinical efficacy in the treatment of LDH(P<0.05),but no studies have shown that needle knife nerve stimulation has poor safety.CONCLUSION:1.The clinical efficacy of acupotomy nerve stimulation in the treatment of LDHR is better than that of conventional Acupotomy.2.In the treatment process of this study,there were no adverse reactions such as hematoma,severe pain and important neurovascular injury,indicating that needle knife nerve stimulation is safe and reliable in the clinical application of LDHR3.Nerve stimulation is effective and safe in the treatment of LDH.
