Clinical Observation Of Weifukang Decoction In Treating Acute Pancreatitis With Hypertriacylglyceremia Of Damp-heat,Turbidity And Stasis

Abstract/Summary:
Objective : Objective To observe the clinical efficacy of Weifukang Fang combined with western medicine in the treatment of hypertriglyceridemic acute pancreatitis(HTGP)in the mild and moderate cases of dampness,heat,turbidity and stasis,and to provide an effective method for the coordinated treatment of HTGP by traditional Chinese and western medicine.Methods: 60 patients with mild and moderate HTGP(damp-heat,turbidity and stasis type)who met the inclusion criteria of this study were randomly divided into control group(n=30)and observation group(n=30).The control group received conventional western medicine treatment,and the observation group received conventional western medicine treatment combined with Weifukang prescription for 1 week.TCM syndromes,physical and chemical indexes(serum TG,TC,AMY,WBC,CRP,D-dimer,arterial blood lactic acid),APACHE-Ⅱ score,BISAP score,MCSI score,relief time of abdominal pain and abdominal distension,recovery time of gastrointestinal function,and length of hospital stay were observed in 2 groups at the 3rd and 7th day of treatment.Results:(1)Clinical efficacy: the total effective rate of the observation group was 96.43%,and the total effective rate of the control group was 89.29%,the difference between the two groups was statistically significant(P<0.05),the efficacy of the observation group was better than the control group.(2)TCM syndrome score:(1)Comparison between groups.On the third and seventh day of treatment in the same period,the total score of TCM syndrome in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05),and the observation group was better than the control group.(2)intra-group comparison showed that both groups decreased on the 3rd and 7th day after treatment compared with before treatment,with statistical significance(P<0.05).(3)TCM syndrome scores:(1)Comparison between groups: on the 3rd and 7th day of the same treatment,there were statistically significant differences between the observation group and the control group in terms of abdominal pain,abdominal distension,nausea and vomiting,dry mouth and bitter mouth,conglutination or obstruction of the stool(P<0.05),the observation group was better than the control group;Except for the third day,there was no significant difference in the tolerance between the two groups(P>0.05).(2)Intra-group comparison:The scores of TCM syndromes in the two groups on the 3rd and 7th days of treatment decreased compared with those before treatment,with statistical significance(P<0.05).(4)Gastrointestinal function recovery time,abdominal pain and abdominal distension relief time: the observation group was shorter than the control group,the difference was statistically significant(P<0.05).(5)Physical and chemical indicators:(1)Comparison between groups: on the 3rd and 7th day of treatment in the same period,the levels of serum TG,WBC,CRP,D-dimer and arterial blood lactic acid in the observation group were significantly different from those in the control group(P<0.05),and the observation group was better than the control group;Except for the third day,there was no significant difference in TC and AMY between the two groups(P>0.05).(2)Within groups,there were significant differences in serum TG,WBC,CRP,D-dimer and arterial blood lactic acid levels between the two groups on day 3 and day 7 compared with those before treatment(P<0.05).There were no significant differences in AMY and TC between the control group and before treatment except for the third day(P>0.05).(6)grading and scoring of acute pancreatitis:(1)Comparison between groups showed that the APACHE-Ⅱ score of the observation group was significantly different from that of the control group on the third and seventh day of treatment in the same period(P<0.05),and the observation group was better than the control group.There was no significant difference in BISAP scores between the two groups after treatment(P>0.05).There was no significant difference in MTSI scores between the two groups on day 3(P>0.05).The score of MCTSI in the observation group was statistically significant compared with the control group after 1 week of treatment(P<0.05),and the observation group was better than the control group.(2)intra-group comparison showed that there were significant differences in APACHE-Ⅱ score and BISAP score between the two groups on day 3 and day 7 compared with that before treatment(P<0.05).There was no significant difference in MTSI scores between the two groups on the 3rd day of treatment and before treatment(P>0.05),and the improvement degree was similar between the two groups.There was significant difference between 1 week of treatment and before treatment(P<0.05).(7)In terms of length of stay,the length of stay in the observation group was significantly shorter than that in the control group(P<0.05).Conclusion:(1)Weifukang prescription combined with western medicine in the treatment of moderate and moderate HTGP patients with damp-heat,turbidity and stasis can effectively improve the clinical syndromes,relieve intestinal paralysis,promote the recovery of gastrointestinal function,shorten the length of hospitalization,and the effect is better than conventional western medicine treatment alone.(2)Weifukang prescription combined with western medicine in the treatment of mild and moderate HTGP patients with damp-heat,turbidity and stasis can effectively reduce serum TG,TC,AMY,WBC,CRP,D-dimer and arterial blood lactic acid levels in the early stage of SIRS,reduce inflammatory reaction and reduce the grading score of acute pancreatitis,and the efficacy is better than conventional western medicine treatment alone.
