| Objective:In the present study,we investigated the efficacy of using Dahuang Zhechong Pill in the clinic to treat varicocele in adolescents,analyzed the mechanism of Dahuang Zhechong Pill treatment and its effect on sperm quality,sex hormone and testicular volume,so as to provide the basis for the effective clinical practice of Dahuang Zhechong Pill.Methods:50 cases of adolescents varicocele patients with kidney deficiency and blood stasis syndrome were selected,including sperm quality,sex hormone,testicular volume abnormalities,oligospermia caused by other factors,or testicular atrophy,or abnormal sex hormone.A statistical random number table method was applied,and the included 50 eligible patients were divided into treatment and control groups according to the division criteria of25 patients in each group.On the basis of eligibility for safety testing,the treatment group was treated with Dahuang Zhechong Pill(Beijing Tongren Tang 3G*10 pill),6 g twice daily.The control group was treated with Aescuven fort,1 tablet twice daily.After 2 months of treatment,the clinical symptom indicators,TCM symptoms,and comprehensive efficacy,changes in varicocele,sperm concentration,motility,total motility,peripheral blood sex hormones(total testosterone,prolactin),testicular volume,and testicular atrophy index were evaluated in both groups.All data were analyzed and processed using spss25.0.Differences were considered statistically significant when p < 0.05.Data from clinical studies were collected by mean standard deviation(x ± s),rank sum test was used for grade data,paired sample t-test was used for within group differences,and independent sample t-test was used for between group differences.Results:after taking medication for 2 months,the clinical observations were as follows: 1.Clinical indicators efficacy assessment indicated that the overall response rate was 83.3% in the treatment group and 82.6% in the control group,which was not significantly different between the two groups(P =0.238 > 0.05).The TCM symptom efficacy assessment indicated that the total effective rate was 70.8% in the treatment group and 60.9% in the control group,which was significantly different between groups(P = 0.034 < 0.05).2.Both the treatment group and the control group improved varicocele effectively.The comparison within the group was statistically significant(P <0.05).There was significant difference between the two groups(P < 0.05).3.The sperm concentration,PR and PR + NP in the treatment group and the control group were significantly improved(P< 0.05).There was no significant difference between the two groups(P> 0.05).4.The treatment group and the control group were effective in improving the level of total testosterone,prolactin and testicular atrophy index,with statistical significance(P < 0.05).There was no significant difference between the two groups(P > 0.05).Conclusions:1.Both Dahuang Zhechong Pill and Aescuven fort can improve the inner diameter of the spermatic vein,relieve the stasis and backflow in the spermatic vein,and reduce the dilatation of the spermatic vein in adolescent patients.Because of its strong ability to improve the velocity of venous blood return,reduce venous pressure and improve the elasticity and contractility of the venous wall.Therefore,in improving the inner diameter of spermatic vein,Aescuven fort is more effective than Dahuang Zhechong Pill.2.Dahuang Zhechong Pill for adolescents with varicocele oligozoospermia treatment exact.It can effectively improve the sperm concentration,sperm motility and so on in patients,which confirms that Dahuang Zhechong Pill can effectively treat adolescents varicocele oligozoospermia.3.The properties of maintaining the stability of sperm microenvironment in adolescents VC testes by Dahuang Zhechong Pill can effectively inhibit the pathological state of the intratesticular tissue structure induced by varicocele in adolescents and repair the oxidative stress damage of testes.4.Both Dahuang Zhechong Pill and Aescuven fort could effectively elevate the total testosterone level in peripheral blood.The mechanism is that under the control of the hypothalamic-pituitary-gonadal axis,the secretion of statin decreases,which effectively increases the total testosterone level in order to maintain the stability of hormone level.It acts synergistically on stromal cells,with a concomitant decline in pituitary prolactin levels,reducing prolactin synthesis,and in the same period achieving the goal of lowering prolactin.5.Dahuang Zhechong Pill have the efficacy of invigorating blood circulation and eliminating stasis and replenishing kidney essence.It can effectively improve the clinical symptoms of testicular pain,scrotal discomfort,and aching of the low back and knee to some extent. |
