| Objective:This study was to observe the changes of TCM syndrome score, quality of life assessment(CAT),lung function(FEV1,FVC,FEV1/FVC),six minute walk test(6MWT),matrix metalloproteinase-9(MMP-9)of patients with chronic obstructive pulmonary disease(COPD)at stable stage(lung and kidney qi deficiency syndrome)treated by modified Pingchuan Guben Decoction before and after treatmentMethods:84 patients with stable COPD(lung and kidney qi deficiency syndrome)were randomly divided into control group and treatment group,42cases in each group. The control group was given conventional western medicine treatment(tiotropium bromide inhalation powder spray daily(18ug/inhalation)),and the treatment group was given conventional western medicine treatment+Pingchuan Guben Decoction(tiotropium bromide inhalation powder spray+Pingchuan Guben Decoction)After regular treatment for 3 months, the changes of TCM syndrome score, cat score, lung function(FEV1,FVC,FEV1/FVC),6MWT and MMP-9 were observed.Results:(1)a total of 84 COPD patients were included in this study, including 2 cases in the control group and 1 case in the treatment group.Finally,40 patients were included in the control group and 41 patients were included in the treatment group. No serious adverse reactions occurred from the beginning to the end of the trial.(2)Before treatment, the age, gender, course of disease,TCM syndrome score, cat score,6MWT,lung function(FEV1,FVC,FEV1/FVC)and MMP-9 of the two groups were compared. It was found that there was no significant difference between the two groups(P>0.05),so it was comparable.(3)Intra group comparison:after 3 months of treatment, the cat score,TCM syndrome score and MMP-9 content of the treatment group and the control group were lower than those before treatment, the difference was statistically significant(P<0.05);the 6MWT,lung function(FEV1,FVC,FEV1/FVC)of the treatment group and the control group were higher than those before treatment, the difference was statistically significant(P<0.05),indicating that the modified Pingchuan Guben decoction Conclusion:combined tiotropium bromide and tiotropium bromide alone can effectively relieve the symptoms of COPD patients in stable stage(lung kidney qi deficiency syndrome),improve lung function and airway remodeling index, and improve the quality of life.(4)Comparison between groups:after 3 months of treatment, the cat score,MMP-9 content and TCM syndrome score(cough, expectoration, wheezing, shortness of breath, fatigue, tinnitus)of the treatment group were lower than those of the control group, the difference was statistically significant(P<0.05);however,there was no significant difference in TCM syndrome score(spontaneous sweating, waist and knee soreness)between the treatment group and the control group(P>0.05).The 6MWT and lung function(FEV1,FVC,FEV1/FVC)of the treatment group were higher than those of the control group, and the difference was statistically significant(P<0.05).It showed that the modified Pingchuan Guben decoction combined with tiotropium bromide had more curative effect and advantages than tiotropium bromide alone, but they had the same curative effect in TCM syndrome score(spontaneous sweating, waist and knee soreness).Conclusions:1、Pingchuan Guben decoction combined with tiotropium bromide in the treatment of stable COPD(lung and kidney qi deficiency syndrome)can effectively improve the symptoms and lung function, and improve the quality of life of patients.2、Conclusion:Pingchuan Guben decoction combined with tiotropium bromide can reduce the content of MMP-9 in COPD patients, which may be one of the mechanisms of protease Antiproteinase imbalance in the treatment of stable COPD(lung kidney qi deficiency syndrome).3、There were no serious adverse reactions in the treatment group and the control group, which proved that the treatment scheme was safe. |
