Objective:The purpose of this study is to explore the relationship between colon adenoma and lipid metabolism,and the possible mechanism of prdx6 protein expression and lipid metabolism level in the development of colon adenoma with three common constitutions:Damp heat,Qi deficiency and yang deficiency Objective to provide reference for the early prevention and treatment of colonic adenoma in patients with predilection constitution of colonic adenoma.Methods:Study 1:a total of 210 patients with colorectal adenoma,proliferative polyp and adenocarcinoma were randomly selected from january2020 to December 2020 in Nanjing Hospital of traditional Chinese medicine for retrospective analysis.142 cases of colorectal adenoma were the study group,68 cases of proliferative polyps were control group,and total cholesterol was analyzed Cholestrol,TC,triglyceride,high density liprotein,low density liprotein(LDL-C),apoliprotein A1,apoliprotein-b,apolprotein-b,apo(b),lipoprotein,LP(a),body mass Index,BMI),to investigate the effect of lipid metabolism on the occurrence and development of colon adenoma..Study 2:Three data sets related to colon adenoma were selected from geo(Gene Expression Omnibus)database,and the intersection of these three data sets was selected,and differential expressed genes(DEGs)were screened out.The enrichment analysis of go(gene ontology)and KEGG(Kyoto Encyclopedia of genes genes)pathway were used to analyze the diversity of these three data sets.Through consulting the relevant literature,prdx6 gene to be studied was selected.Study 3:randomly selected from June 2020 to December 2020 in Nanjing Hospital of traditional Chinese medicine for colonoscopy and pathological diagnosis of colon adenoma,hyperplastic polyps and colon adenocarcinoma,20 cases of TCM Constitution for damp heat were included in study group 1,20 cases of qi deficiency were included in study group 2,and 20 cases of yang deficiency were included in study group 3 The control group included 20 patients with polyps and mild constitution.The expression of prdx6 protein in the above four groups was detected by immunohistochemistry,and the correlation between them was discussed.Results:1.In Study 1,the P values of gender,age and BMI of colon adenoma group and hyperplastic polyp group were all greater than 0.05,with no significant difference;the P values of TC,TG,LDL-C,ApoA1,apoB and LPA between the two groups were all greater than 0.05,with no significant difference;the p value of HDL-C was less than 0.05,with statistical difference.2.In Study 2,data mining using bioinformatics showed that prdx6 may be a key differentially expressed gene related to lipid metabolism in colon adenomas.3.In Study 3,there were no significant differences in gender,age and BMI between the patients with colon adenoma of different constitution and the control group(P>0.05).There were differences in the levels of TC,HDL-C,LDL-C and ApoA1(P<0.05),but there were no significant differences in the levels of TG,apoB and LPA(P>0.05).The positive rate of prdx6 was Yang deficiency(95%)>Qi deficiency(75%)>damp heat(55%),P=0.000<0.01.By comparing the positive rate of prdx6 between groups,the difference was statistically significant(P=0.034<0.05)in Yang deficiency group and damp heat group;the difference was statistically significant(P=0.101>0.05)in Yang deficiency group and Qi deficiency group,and the difference was not statistically significant(P=0.620>0.05)in damp heat group and Qi deficiency group.Conclusions:1.There is a certain correlation between the occurrence of colonic adenoma and the level of lipid metabolism,especially those with abnormal HDL-C may be more likely to develop colonic adenoma.2.Prdx6 may be a potential target for the diagnosis and treatment of colonic adenoma.3.The levels of TC,HDL-C,LDL-C and ApoA1 were different in patients with colon adenoma of different constitution.4.The positive rate of prdx6 in colonic adenoma was higher than that in proliferative polyp.5.The positive rate of prdx6 protein may be related to constitution.The positive rate of damp heat constitution is higher than that of Yang deficiency constitution.
