Abstract/Summary:
Objective:This study aims to explore the distribution of TCM constitutions in PCOS patients, the differences in general clinical characteristics, external manifestations, and lipid metabolism indicators among patients with different constitutions, and the correlation between constitution and lipid metabolism,so as to improve the relevant indicators of patients through "discrimination and conditioning of constitution" and shed new light on the early prevention and treatment of PCOS.Methods:From January 2014 to January 2023,a total of 1195 patients who were diagnosed with PCOS and provided complete information were included in the integrated clinical research information system of the Gynecology Clinic of the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine.First of all,we studied the distribution laws of TCM constitution and lipid metabolism.Secondly,we divided the included patients into nine groups according to the TCM constitution classification and judgment standard,i.e.yang-deficiency,yin-deficiency,qi-deficiency,phlegm-damp,damp-heat,blood-stasis,special-endowment,qi-stagnation and peace, and compared the indicators among the groups. Finally,we determined whether there was a correlation between TCM constitution and other indicators in PCOS patients through Spearman correlation analysis.Results:1.The dominant constitutions of the 1195 PCOS patients included in this study are peace(29.5 4%),qi-deficiency(2 3.93%),blood-stasis(12.89%),damp-heat(8.45%),qi-stagnation(8.28%),phlegm-damp(6.19%),yang-deficiency(5.44%),yin-deficiency(4.10%),and special-endowment(1.17%).2.Among the PCOS patients included in this study,31.21%were overweight and 32.22%were obese; In all,59.92%were central obese patients.3.In this study, there were differences in general clinical features, external signs and lipid metabolism indexes among different TCM constitutions samples of PCOS patients(P<0.05).4.In this study, peace was negatively correlated with age, body weight,BMI,WC,WHR,WHtR,acne score,TG,TG/HDL,TC/HDL,LAP,and VAI(r<0,P<0.05),and positively correlated with SBP(r>0,P<0.05).5.Qi-deficiency was positively correlated with body weight,BMI,WC,WHR,WHtR,TG,APO-A,APO-B,LDL/HDL,TG/HDL,TC/HDL,LAP,and VAI(r>0,P<0.05),and negatively correlated with hairy score(r<0,P<0.05).6.Blood-stasis was negatively correlated with SBP and hairy scale(r<0,P<0.05),and positively correlated with WHR,acne scale and APO-A(r>0,P<0.05).7.Damp-heat had a negative correlation with WHtR(r<0,P<0.05)and a positive correlation with hairy score and acne score(r>0,P<0.05).8.Phlegm-damp positively correlated with SBP,body weight,BMI,WC,HC,WHR,WHtR,acanthosis nigricans score, acne score,TG,APO-B,LDL/HDL,TG/HDL,TC/HDL,LAP,and VAI(r>0,P<0.05),and negatively correlated with APO-A(r<0,P<0.05).9.Yang-deficiency was negatively correlated with body weight,BMI,WC,WHR,WHtR,acanthosis nigricans score,TG,APO-B,LDL/HDL,TG/HDL,TC/HDL,LAP,and VAI(r<0,P<0.05),and positively correlated with hairy score(r>0,P<0.05).10.Qi-stagnation,yin-deficiency,and special-endowment are not related to general clinical features, external signs and indexes of lipid metabolism.Conclusions:1.The dominant TCM constitutions of PCOS patients are peace,qi-deficiency,blood-stasis,damp-heat,and qi-stagnation.2.Most PCOS patients suffer from obesity, and most of them are central obese.3.TCM constitutions of PCOS patients are related to lipid metabolism indicators. Therefore, adjusting biased constitutions can alleviate related symptoms.
