| Objective: This study aims to explore the role of probiotics in the treatment of feeding intolerance in premature infants,to guide the treatment of feeding intolerance, and to explore the potential inherent between probiotics, feeding intolerance and gastrin secretion levels Contact, looking forward to discovering early biological indicators of feeding intolerance. Methods: The premature infants who were hospitalized in the neonatology department of our hospital from September 2019 to December 2020 were collected as the research objects.According to the relevant literature and the design of this study,GPower was used to calculate the sample size(ω=0.3,α=0.05,1-β=0.9,Df=1),the sample size is117.Based on the consideration of reliability and prudence,a total of 133 samples were included in the study on the basis of professional judgment and informed consent of the family members.Among them,58 samples were included in the experimental group, that is,routine treatment was added with bifidobacteria triple viable bacteria(Pefekang)orally, and 75 samples were included in the control group(conventional treatment without probiotics). The incidence of feeding intolerance of the two groups of children was observed, and the gastrin levels within 24 hours and 1 week of birth of the children in the group were detected. The SPSS16.0 statistical software package was used for processing, the measurement data was expressed as mean ± standard deviation, and the count data was tested by test, and P<0.05 was considered statistically significant. The influencing factors were analyzed by Logistic regression using the Enter method, and the receiver characteristic curve(roc)was constructed to analyze the statistical connection. Results: A total of 44 cases of feeding intolerance occurred in the two groups, the incidence was 33.1%,the incidence of feeding intolerance in the experimental group was 22.41%,and the incidence of feeding intolerance in the control group was 41.33%,and there were statistics between the two School difference(P=0.021).The gastrin level of the experimental group within 24 hours was 2.82±0.50 pmol/L,the gastrin level of the control group was 2.73±0.32 pmol/L within 24 hours, and there was no statistical difference in the 24-hour gastrin level between the experimental group and the control group(P=0.219).There was a statistical difference in gastrin levels between the experimental group and the control group in 1 week(P=0.000).The gastrin level of the experimental group was higher than the control group in 1 week. There was no statistical difference between the feeding intolerance group and the feeding tolerance group in the gastrin level index in 1 week. The increase in gastrin levels in the feeding intolerance group and the feeding tolerance group in the experimental group was statistically different from that in the feeding intolerance group and the feeding tolerance group in the control group. The increase of gastrin in the feeding intolerance group or the feeding tolerance group was higher than that of the control group. However, comparing the two groups,we found that whether it is the experimental group or the control group, the gastrin level within 24 hours of the feeding intolerance group and the feeding tolerance group are statistically different, and the gastrin level within 24 hours of the feeding intolerance group They were all lower than the feeding tolerance group.Logistic regression analysis was performed using the Enter method, and it was found that the 24-hour gastrin level was correlated with feeding intolerance, and the low person was more likely to have feeding intolerance. On this basis,we draw the roc curve, the cut-off value is 2.74,the area under the curve is 0.661,P=0.003,which is statistically significant. Conclusion: 1.Probiotics help to improve the feeding tolerance of premature infants and reduce the incidence of feeding intolerance, and prebiotics have a positive effect on the gastrointestinal motility of premature infants and contribute to the secretion of gastrin.2.Statistics found that there is a certain correlation between 24-hour gastrin level and feeding intolerance, and it has a certain predictive value for feeding tolerance.Premature infants with low 24-hour gastrin levels are more likely to have feeding intolerance. |
