Background:NUMB was originally thought to be the determinant of cell fate in Drosophila development.It was later proved to play a role in the control of asymmetric cell division,cell fate selection,cell migration,cell adhesion and other physiological development processes.It also plays a role in many signal transduction pathways such as Notch,Hedgehog,and p53.The latest research shows that there are reports of abnormal expression of NUMB in cancers,such as breast cancer,non-small cell lung cancer,salivary gland cancer,and cervical squamous cell carcinoma.The analysis of differential gene expression profiles related to acute myocardial infarction conducted by our group in the early moment showed that NUMB gene is lower expressed in peripheral blood of patients with acute myocardial infarction(AMI).Objective:This experiment verified the differential expression of NUMB gene in peripheral blood by expanding the sample size,and further evaluate whether in peripheral blood the lower NUMB gene can be acted as a genetic molecular marker for stable coronary heart disease(CAD)patients to develop into acute myocardial infarction.Combined with the existing research progress of NUMB gene,further speculate its role in the occurrence of acute myocardial infarction.Methods:According to the latest international diagnostic standards,124 patients with AMI who were hospitalized in the Department of Cardiology in The Third Hospital of Jilin University were collected as the experimental group,and 115 patients with SCAD were the control group.Collected the peripheral blood lymphocytes of the patients,analyzed and compared the clinical data between the two groups,and detected the expression difference of NUMB gene mRNA level in the peripheral blood of the two groups by real-time quantitative polymerase chain reaction(RT-PCR)method.Results:The results of clinical baseline data analysis of the subjects in the AMI group and SCAD group displayed: There was no statistically different in gender,history of hypertension,history of smoking,diabetes history,serum triglycerides(TG),serum total cholesterol(TC),and high-density llipoprotein cholesterol(HDL-C)and low-density lipoprotein cholesterol(LDL-C).The AMI group was significantly older than the stable CAD group(P=0.020),and the difference was obviously significant.The fasting blood glucose level of the AMI group was clearly higher than the stable CAD group(P=0.012),and the divergence was obviously significant.Comparative analysis of the relative expression of NUMB gene mRNA level between the AMI group and the stable CAD group displayed : AMI group have a lower expression of NUMB gene in peripheral blood than stable CAD group(z=-1.985,p=0.047)in mRNA level,and its relative expression was 0.932 times that of stable CAD group.Further analysis of the relationship between clinical data and NUMB gene mRNA level displayed : The relative expression of NUMB gene in mRNA level was unrelated with age(z=-0.461,p=0.645)and fasting blood glucose level(z=-0.596,p=0.551).Multiple logistic regression analysis showed: In peripheral blood the lower NUMB gene expression is an independent risk factor for the occurrence of stable CAD patients to AMI.Compared with the higher NUMB gene group,the NUMB gene low expression group had a 3.287 times higher risk of AMI.Older age is also an independent risk factor for AMI,and the risk of AMI is 1.853 times.Elevated fasting blood glucose level is not an independent risk factor for AMI.Conclusions:NUMB gene is significantly low expression in the peripheral blood of AMI patients,and the low NUMB gene is an independent risk factor for AMI.The low NUMB gene in peripheral blood can be a genetic molecular marker for early warning of AMI in patients with stable CAD. |
