| Objective To summarize the basic characteristics of febrile seizure(febrile seizure,FS)in children in Dali area,the difference of related factors between one and more than 2 times of febrile seizure in a single course of febrile disease,and the risk factors of FS recurrence.Methods The clinical data of 167 children with FS diagnosed for the first time in the first affiliated Hospital of Dali University from January 1,2015 to December 31,2019 were analyzed retrospectively.And the recurrence was recorded during the follow-up period to December 31,2020.All data were processed by SPSS21.0 statistical software.Results 1.Among the 167 cases of FS,95 were boys(56.9%)and 72 girls(43.1%).The minimum age of first onset was 6 months and the maximum was60 months,with an average of(25.65 ±12.84)months.Among them,15 infants(9.0%),115 infants(68.9%),and 37 preschool children(22.1%).Before convulsion,fever time ≤ 24 hours was found in 136 cases(81.4%),24h-48 h in 29 cases(17.4%)and 48h-72 h in 2 cases(2.2%).The duration of first convulsion was ≤ 5min in 141cases(84.4%),5min-15 min in 21 cases(12.6%),15min-30 min in 4 cases(2.4%),and > 30 min in 1 case(0.6%).There were 166 cases of generalized seizures(99.4%)and 1 case of focal seizures(0.6%).2.In the course of single fever,one convulsion occurred in 134 cases(80.2%),and convulsion ≥ 2 times in 33 cases(19.8%).There was significant difference in the age of first onset between the two groups(≤ 18 months,>18 months)(P < 0.05).There was no significant difference in body temperature,duration of convulsion,family history of FS,family history of epilepsy,history of perinatal asphyxia,onset season and abnormal rate of EEG(P > 0.05).3.167 children with FS were followed up to December 31,2020,no recurrence occurred in 111cases(66.5%)and recurrence in 56 cases(33.5%).In single factor analysis,recurrence was correlated with body temperature ≤38.0C,family history of FS,complex febrile seizure,fever time ≤ 1h before convulsion and abnormal EEG.Multivariate analysis further found that age of first onset ≤ 18 months,body temperature at the time of convulsion ≤ 38.0C,febrile time before convulsion ≤ 1h,family history of FS and abnormal EEG were independent risk factors for recurrence of FS.Conclusion 1.FS is more common in young children,boys than girls,convulsions often occur within 24 hours of fever,most of them are generalized seizures,and most of them are single seizures in a single febrile course,and the duration of seizures is less than 5 minutes.There is no significant difference in the onset season of FS in this area,but there is obvious regionality in the onset season.2.There is a difference in the age of first onset(≤ 18 months,> 18 months)between one convulsion and ≥ 2 convulsions in a single course of febrile disease.The age of onset ≤ 18 months is more likely to have more than 2convulsions in a single course of febrile disease.3.Age of first onset ≤ 18 months,body temperature ≤ 38.0C at the time of convulsion,fever time ≤ 1h before convulsion,family history of FS and abnormal EEG are the risk factors for recurrence of FS. |
