| Objective:To observe the effect of treatment on the clinical symptoms of patients through the clinical study of Yichangtongmi Decoction combined with Chinese medicine directed drug penetration therapy for chronic constipation of yang deficiency of the spleen and kidney,to objectively evaluate its clinical efficacy,and to explore the combination of internal and external treatments of traditional Chinese medicine.The mechanism of treating chronic constipation with spleen and kidney yang deficiency provides clinical evidence and data support for combined treatment.Methods:According to the inclusion and exclusion criteria,a total of 74patients with constipation of spleen and kidney yang deficiency type who attended the Anorectal Department of Hubei Provincial Hospital of Traditional Chinese Medicine from September 2018 to January 2020 were selected.The patients were divided into two according to a random number table.Group:37cases in the treatment group(treatment with Yichangtongmi Decoction combined with directional drug penetration therapy of traditional Chinese medicine),and 37 cases in the control group(treatment with lactulose oral liquid).The two groups of patients were compared in terms of general information(gender,age and course of disease),clinical efficacy,defecation indicators,clinical symptoms of traditional Chinese medicine,etc.,P>0.05,the difference was not statistically significant and comparable.The two groups of patients were treated for 4 weeks,and the observation indicators after the treatment were counted;and 4 weeks after the drug was stopped,the recurrence was counted,and the clinical efficacy of each group was finally evaluated.Results:(1)The first three groups of patients were tested in terms of gender,age and disease course,and the differences were not statistically significant and comparable(P>0.05).(2)Comparison of clinical efficacy:Of the 37patients in the treatment group,15 were cured,10 were markedly effective,7were effective,and 5 were ineffective,with a total effective rate of 86.49%;among 37 patients in the control group,5 were cured,11 were markedly effective,and 8 were effective.Cases,13 cases were invalid,and the total effective rate was 64.86%.After 4 weeks of treatment,the data of the two groups were tested by Chi-square test,χ~2=4.158,P=0.030<0.05,the difference was statistically significant.(3)Comparison of defecation indicators:The four indicators of defecation and clearing conditions between the two groups were compared with those before treatment,and the differences were statistically significant(P<0.05);the treatment group had significant differences with the control group in terms of improving defecation time and feeling of incomplete defecation after defecation(P<0.05).(4)Comparison of clinical symptom indexes of Chinese medicine:Compared with before treatment,both groups can significantly improve the four indexes of Chinese medicine clinical symptoms(P<0.05),and the effect of the treatment group is better than that of the control group,and the difference is statistically significant(P<0.05).(5)Comparison of mental and psychological scores:Compared with before treatment,both groups can significantly improve the two mental and psychological scores(P<0.05),and the effect of the treatment group is better than that of the control group,and the difference is statistically significant(P<0.05).(6)Recurrence:4 weeks after stopping the drug,5 of the 32 effective patients in the treatment group relapsed,with a recurrence rate of 15.63%;15of the 24 effective patients in the control group recurred,with a recurrence rate of 62.50%.The data of the two groups were tested by chi-square,χ~2=6.094,P=0.014<0.05,the difference was statistically significant.Conclusion:(1)The theory of"warming the yang to invigorate the spleen,moisturizing the intestines and laxative"of Chinese medicine has guiding significance in the treatment of chronic constipation due to deficiency of spleen and kidney yang in clinical practice.(2)Yichangtongmi Decoction combined with TCM directed drug penetration therapy can effectively treat chronic constipation of spleen and kidney yang deficiency.(3)Yichangtongmi Decoction combined with TCM directional drug penetration therapy to treat spleen and kidney yang deficiency chronic constipation is an effective,simple and convenient treatment method with low recurrence rate,which is worthy of clinical application. |
