Objective:The research purpose of this experiment is to investigate the use of autogenous teeth graft material prepared by the extracted tooth after minimally invasive tooth extraction in patients with periodontal disease,which is used in the alveolar ridge preservation therapy,as well as the subsequent implant treatment comparison of clinical efficacy.Methods:1.The experimental object were 1-year-old adult male beagle dogs.Physical and mechanical stimulation was used,supplemented by diet quality and quantity control,to establish an animal experimental model of periodontal disease,which simulated the similar oral environment of periodontal patients.2.Using the second to fourth premolars of the beagle mandible as the experimental sites,the isolated teeth were collected after minimally invasive extraction,and the AutoDenty tooth powder grinding instrument was used to prepare the autogenous teeth graft material.AUTG material used in clinic was obtained through some steps which contained disinfection,grinding,the acid etching demineralization,degreasing and neutralization.Set up the control group without bone meal transplantation,the Bio-oss group implanted with a heterogeneous bone substitute that was widely used clinically,and the AUTG group that was mainly verified in this experiment.3.Using cone-beam CT to record the three-dimensional imaging changes of the alveolar ridge immediately after surgery,4 months after surgery,and 6 months after surgery,as well as the maintenance of attached gingiva,to evaluate whether AUTG had a good quality of bone substitute materials,and compare the superiority of xenogeneic bones such as Bio-oss.4.Beagle dogs with a bone healing period of 6 months were used for repair-oriented personalized implant treatment.After 3 months of osseointegration period,the assessment was based on whether the formation of osseointegration,the implant torque≥35N·m,and the absence of peri-implantitis as the standard,and qualified implants were included in successful cases to calculate the survival rate of implants.Cone-beam CT was used to record the changes in bone loss at the mesial and distal edges of the implant to prove whether the AUTG group had an advantage in later implantation.5.Calculate the implants-osseointegration rate of the implants in different groups through paraffin sections of the alveolar bone during the healing period of 3 months,and compare whether the implants-osseointegration rate of the AUTG group was higher than that of the Bio-oss group and the control group.Results:1.Through physical and mechanical stimulation of periodontal tissues,supplemented by viscous high-sugar diet,Beagle dogs’ mandibular alveolar bone exploration and 1~2mm attachment loss and tooth looseness Ⅰ°-Ⅱ° after 12 weeks indicated that we successful established an animal experimental model of periodontal disease.2.At 4 months after the preservation of the alveolar ridge site,the three-dimensional changes of the alveolar ridge of the cone-beam CT were measured.The maintenance effect of hard tissue in the AUTG group was better than that of the control group.However,there was no difference in the change of central bone height between the AUTG group and the Bio-oss group.3.At 6 months after the preservation of the alveolar ridge site,the difference in the three-dimensional length of the alveolar ridge was not statistically significant.In terms of the degree of soft tissue changes,the maintenance effect of the AUTG group had no significant difference from the other groups.4.After 3 months of implant healing period,under strict screening,the implants of the three groups were all satisfied to be included in the successful cases,and there was no significant difference in the survival rate of implants.Only the distal marginal bone loss in the AUTG group was less than that of the control group,and other comparisons were not statistically significant.5.After 3 months of implant healing period,on the tissue paraffin sections,we found the implant osseointegration rate of the AUTG group was not statistically different from other protocols.Conclusion:AUTG is used as a bone substitute material for alveolar ridge site preservation surgery.It has the equivalent of bone conduction and osteoinduction of allogeneic bone,which helps periodontal patients to maintain the level of hard tissues and the reqcuirement of implant.
