| Objective:By planting surgery patients with insufficient bone mass applying new bone graft material——autologous tooth bone graft material,Preliminary clinical observation of autologous tooth bone graft material to repair bone defect in dental implant surgeryMethods : From September 2015 to February 2017,the patients were treated in the implant center of the Affiliated Stomatological Hospital of Dalian Medical University,Select 7 patients with available teeth(such as the teeth of the third molars,supernumerary teeth,or teeth that could not be treated by severe periodontal disease)and bone mass in the implant area.To understand the age,sex,missing teeth,the size of the gap,the gingival condition,etc.2 cases of anterior teeth area.Posterior maxilla defect in 2 cases,mandibular posterior region deletion in 3 cases.All patients were treated with autogenous bone graft material in implant surgery,at the same time,also accepted the different types of bone increment operation(in some cases,including one or more surgical procedures),including the bone splitting,sinus elevation,guided bone regeneration(PRF)etc.A total of 12 implants(OSSTEM 11 implants,THOMMAN 1 implants)were implanted at the same time or late in the implant.After 3-6 months,prosthesis was performed.Regular reexamination,examination by CBCT,radiographs,periapical film images to investigate the changes of bone mass at the same site around the implant before and after 3 months of operation and after operation for 6 months;To check whether there is any postoperative complication;Observe the gingival condition around the prosthesis;To understand the patient's satisfaction with the operation process,the use function of the prosthesis and the appearance.Results:(1)All of the cases were wounds healing well,no postoperative infection,no inflammatory exudation and no nerve injury,no damage,no adjacent tooth jaw fracture and other complications.(2)There was no shedding of the 12 implants with a retention rate of 100%.(3)The preoperative average bone defect area of alveolar bone height was 8.09±2.87 mm,the width is 4.94±1.97 mm,3 months after surgery,average bone graft height width is 14.80 ± 2.03 mm,6.88 ±0.73 mm,6 months after the operation and the average bone height was 14.45 ± 1.99 mm,the width is 6.62±0.66 mm,after operation,two groups of data were significantly improved,the difference was statistically significant(P<0.05);compared with 3 months after operation,bone graft 6 months after operation there is st ill a small amount of bone resorption,but no statistically significant difference(P > 0.05);(4)plaque and calculus index detection,one cases of postoperative patients with poor oral hygiene maintenance of grade 2,has been on the oral health education.(5)in the 7 patients,there were 2 patients who were afraid of the surgical procedure,Other patients showed a good feeling during the operation,for postoperative discomfort,only 1 cases of patients with mild edema after surgery,conventional antibiotic treatment gradually reduced and improved,and the patient can tolerate.All patients were satisfied with the masticatory function of the prosthesis,only one patient due to occlusal tight,the tooth elongation,advise patients with metal occlusal porcela in crown,because in the posterior region,patients could accept,basically satisfied with the repair effect.Conclusion: The clinical study shows that patients with insufficient bone mass applying new bone graft material——autologous tooth bone graft material after surgery can achieve better clinical results.The alveolar bone height and width were improved after operation,the indications for the treatment of bone defects were enlarged,and the success rate of planting was increased.Preparation of auto logous bone graft material in chairside,saves time and avoids the disadvantages of other bone graft materials such as : need for additional material and immune risks and other shortcomings,to alleviate the economic pressure on patients.The present study shows that autogenous tooth bone is a safe and effective material for bone transplantation,but its long-term effects still need long time observation and study further. |
