Research On Professor Ding Shuwen’s Prescription And Clinical Curative Effect On Atrial Fibrillation Based On Data Mining

Objective:Collect and sort out the medical records of Professor Ding Shuwen’s treatment of atrial fibrillation,through data mining on the medical records data,to explore the law of prescription compatibility.Through the analysis of the efficacy of the case data,observe the clinical efficacy of Professor Ding’s treatment of atrial fibrillation,sum up Professor Ding’s clinical experience in treating atrial fibrillation,syndrome differentiation thinking and academic thinking.Methods : 1.Patients with Atrial fibrillation who were treated by Professor Ding Shuwen from the Department of Cardiovascular Diseases of Shandong Provincial Hospital of Traditional Chinese Medicine from January2018 to September 2019 were collected,according to the inclusion and exclusion criteria,the general situation of patients was entered into an Excel form,and then entered into the traditional Chinese medicine inheritance assistance system(V2.5)software,through the cluster analysis to obtain the corresponding prescription combination of different syndrome types.2.Observe the changes after 4 weeks of treatment with Professor Ding’s Chinese medicine in average number of attacks and duration of atrial fibrillation,maximum duration of P wave(Pmax)and P wave dispersion(Pd),average heart rate,number of premature beats,left atrium inner Diameter,left ventricular ejection fraction and TCM symptoms,and regular follow-up after 12 weeks and 24 weeks to summarize Professor Ding’s experience in treating atrial fibrillation.Results : 1.Among the 144 patients with atrial fibrillation in this study,86 males and 58 females,61～70 years old as the peak age group,suggesting that middle-aged and old men are the main symptoms.Among the top 10 symptoms,palpitation,chest tightness,and shortness of breath were the main symptoms,and fatigue,insomnia,sweating,dry mouth,chest pain,dizziness,and loose stools were the symptoms.Professor Ding’s treatment is mainly based on tonic deficiency medicine,heat-clearing medicine,and blood circulation and blood stasis medicine.There are 4types of Traditional Chinese Medicine syndromes.Among them,the AF of Yin deficiency and fire type is mainly treated by adding and subtracting Danggui Liuhuang Decoction,the AF of Qi deficiency and blood stasis type is mainly treated by adding and removing Huang Qi Er Hao Fang,the AF of Qi and Yin deficiency is mainly treated by adding and subtracting Zhigancao Decoction,and the AF of phlegm-heat disturbing heart type is mainly treated by adding and removing Huanglian Wendan Decoction.2.Through the analysis of efficacy,we can draw: After 4 weeks of treatment,the average number of attacks decreased,the average duration is shortened,Pmax and Pd decreased,the average heart rate decreased,and the number of premature beats decreased,the TCM symptom of patients all improved significantly(P<0.05),and the total curative effect rate of TCM is 71.55%,however the left atrial diameter and left ventricular ejection fraction did not improve significantly(P>0.05).After 12 weeks,and 24 weeks of regular follow-up,it was found that Chinese medicine can significantly reduce the number of paroxysmal atrial fibrillation attacks and have a positive effect on shortening the duration of persistent atrial fibrillation attacks,and most patients insist on taking Chinese medicine daily for about 12 weeks,and then take the medicine intermittently to maintain treatment,indicating that Chinese medicine has a good effect on atrial fibrillation.Conclusions : 1.Professor Ding divided atrial fibrillation into Yin deficiency and fire-boost type,Qi deficiency and blood stasis type,Qi and Yin deficiency type and phlegm-heat disturbing according to Chinese Medicine syndrome types,and treated them with addition and subtraction of Danggui Liuhuang Decoction,addition and subtraction of Huang Qi Er Hao Fang,Zhigancao Decoction and Huanglian Wendan Decoction.2.Professor Ding treats atrial fibrillation according to different syndromes can effectively reduce the number of attacks,shorten the duration of the attack,reduce Pmax and Pd,lower the average heart rate,and reduce the number of premature beats,improve the clinical symptoms of patients,indicating that Chinese medicine has a definite therapeutic effect on atrial fibrillation.
