| Objective: 1.The purpose of this study is to observe the expression characteristics of TH1,TH2 cytokines TNF-α,IL-10 in serum and endometrium of patients with kidney deficiency and blood stasis syndrome(RIF)and analyze their correlation,so as to explore the immunological pathogenesis of RIF.2.To explore the immunological mechanism of Bushen Huoxue decoction from the perspective of TH1/TH2 cytokine imbalance.Method: 30 cases of RIF with kidney deficiency and blood stasis were selected as RIF kidney deficiency and blood stasis group(observation group)and 30 normal fertile women in the same period as the control group were used to count the age,duration of infertility,type of infertility,body mass index,basic hormone level and other general conditions of the two groups.The levels of TNF-αand IL-10 in serum and endometrium of the two groups were measured by chemiluminescence and immunofluorescence respectively at the implantation window of embryo,and the ratio of the two groups(TNF-αand IL-10)was calculated TNF-α/IL-10),the expression characteristics of TNF-α and IL-10 in serum and endometrium of RIF patients with kidney deficiency and blood stasis syndrome were compared and analyzed;after the first collection of samples in the observation group,two menstrual cycles were treated with Bushen Huoxue Decoction,the changes of TNF-α,IL-10 and TNF-α/IL-10 ratio in serum and endometrium of RIF patients before and after the treatment were observed,as well as the improvement of TCM symptoms.Results: 1.Comparison of general conditions: there was no statistical difference between the observation group and the control group in terms of age,infertility years,infertility type,body mass index,basic hormone level(P>0.05);2.Comparison between the observation group and the control group in terms of TNF-α,IL-10 and TNF-α/IL-10 before treatment with traditional Chinese medicine: The levels of serum and endometrial TNF-αin the observation group were significantly higher than those in the control group(P In the endometrium,TH1 cell factor(TNF-α)was dominant expression,while TH2 cell factor(IL-10)was low expression,and the ratio of TNF-α/IL-10 was increased.4.The changes of TNF-α,IL-10 and the ratio of TNF-α/IL-10 in the observation group before and after the treatment of traditional Chinese medicine:(1)in the serum: after the treatment of traditional Chinese medicine,the level of TNF-α in the observation group was significantly lower than that before the treatment(P<0.05),and the level of IL-10 was slightly higher than that before the treatment,But there was no statistical difference(P>0.05),the ratio of TNF-α/IL-10 was significantly lower than before treatment(P < 0.05);(2)in endometrium:TNF-α level was significantly lower than before treatment(P<0.05),IL-10 was no significant change(P>0.05),the ratio of TNF-α/IL-10 was significantly lower than before treatment(P<0.05);5 After treatment,the symptoms of kidney deficiency and blood stasis were improved,and the syndrome score was significantly reduced(P<0.05).Conclusion: 1.There is a dominant expression of TH1 cytokines(TNF-α)in the embryo implantation window of RIF patients with kidney deficiency and blood stasis syndrome.It is speculated that the overexpression of TH1 cytokines may lead to excessive immune rejection of the mother,which may affect embryo implantation and participate in RIF.2.The expression trend of TH1 / TH2 cytokines(TNF-α,IL-10)in serum and endometrium of RIF patients with kidney deficiency and blood stasis syndrome is the same.In the evaluation of maternal TH1 / TH2 cell immune balance,or peripheral serum cytokines can be used instead of endometrium to reduce trauma.3.Bushen Huoxue decoction may play a therapeutic role by down regulating the level of TH1 cell inflammatory factor(TNF-α),correcting the imbalance of TH1/ TH2 cell immunity and inflammatory state. |
