Study On The Correlation Between TSH Level And Carotid Intima-Media Thickness And Blood Stasis In Elderly Subclinical Hypothyroidism Patients

Abstract/Summary:
Objective:In this study,by observing the correlation between TSH levels and carotid intima-media thickness in elderly patients with subclinical hypothyroidism,the correlation between the TSH level in the elderly patients with subcl inical addition and subtraction and the severity of blood stasis syndrome in traditional Chinese medicine,and further analyzed the relevant factors of carotid intima-media thickness and the influencing factors of blood stasis quantitative score,so as to provide reference for clinical diagnosis and treatment.Methods:Select 102 patients with subclinical hypothyroidism that meet the diagnostic criteria,collect relevant data,establish a database,and enter the relevant data into the data table.By measuring the carotid intima-media thickness of patients,using SPSS22.0 statistical software The data were analyzed by one-way ANOVA,and the data that did not meet the normal distribution were tested by non-parametric analysis.Spearman correlation analysis was used to perform multiple regression analysis with carotid intima-media thickness and blood stasis quantitative score as dependent variables.Results:(1)In this study,102 patients(aged subclinical hypothyroidism patients)were included in the study,with an average age of 77.30 ± 9.12 years,including 50 male patients,78.38 ± 9.30 years old,52 female patients,76.27 ± 1.99 years old.There was no significant difference in age between men and women.(P=0.144>0.05 by nonparametric test).(2)Spearman correlation analysis was performed on thyroid function indicators in laboratory indicators,CIMT,and blood stasis quantitative scores:FT3 was negatively correlated with age(P<0.001),CIMT(P=0.009),and blood stasis quantification score(P = 0.013).FT4 was negatively correlated with age(P=0.043)and HDL(P = 0.014).Positively correlated with TG(P=0.021)and TC(P=0.011).Because TSH distribution does not follow a normal distribution,TSH is calculated after logarithmic conversion.LnTSH was positively correlated with age(P=0.009),Hcy(P=0.025),TC(P=0.002),LDL(P = 0.0333),CIMT(P<0.001),and blood stasis quantitative score(P<0.001).(3)The Spearman correlation analysis and regression analysis of CIMT with laboratory indicators showed that:CIMT was negatively correlated with FT3(P=0.009).Positively correlated with age(P<0.001),LnTSH(P<0.001),Hcy(P=0.002),FBG(P=0.037),TC(P=0.010),and blood stasis quantitative score(P<0.001).CIMT was used as the dependent variable for multiple linear regression analysis.Age,LnTSH,and FBG were the factors affecting CIMT,and LnTSH was the most relevant.(4)Comparison of blood stasis group and non-blood stasis group:LnTSH,FT3,CIMT,FT4 were tested by two samples t test,P<0.05,the difference was statistically significant.There was no significant difference in UA between the two groups,P=0.05.Among them,the blood stasis group CIMT(1.22±0.09)>the non-blood stasis group CIMT(1.13± 0.09);the blood stasis group LnTSH(1.79±0.22)>the non-blood stasis group LnTSH(1.63±0.15);the blood stasis group FT3(4.25 ±0.83)<FT3 in non-blood stasis group(4.70±0.87).Age,BMI,Hcy,FBG,TG,TC,HDL,LDL were tested by non-parametric Kruskal-Wallis test,and it was concluded that age was significantly different between the two groups,P<0.001,TC was significantly different between the two groups,P=0.035(5)Correlation analysis of quantified blood stasis scores with laboratory indicators and CIMT showed:The quantitative score of blood stasis was positively correlated with LnTSH(P<0.001),age(P<0.001),CIMT(P<0.001),TC(P=0.003),LDL(P=0.005).The quantitative score of blood stasis was negatively correlated with FT3(P=0.013).(6)Quantitative blood stasis score was used as the dependent variable.Multiple regression analysis showed that:Age,TC,and LnTSH were significant factors affecting the blood stasis quantitative score.Among them,age is the most relevant.Conclusion:(1)There was a positive correlation between LnTSH and CIMT,and the correlation was significant.The higher the TSH level,the greater the likelihood of atherosclerosis.(2)In the comparison of related laboratory indexes and CIMT between the blood stasis syndrome group and the non-blood stasis syndrome group,the CIMT,LnTSH and TC of the blood stasis syndrome group were significantly higher than those of the non-blood stasis syndrome group.(3)LnTSH was positively correlated with blood stasis quantification in elderly patients with SCH.Elderly people with SCH who have high blood stasis should monitor TSH changes.Whether the method of promoting blood circulation and removing blood stasis can improve the TSH value needs further study(4)In addition to blood lipids,age,and FBG that affect CIMT,LnTSH is also a risk factor.
