| Objective:To compare the clinical efficacy of Jin jin and Yuye puncture bloodletting and cold stimulation therapy in the treatment of swallowing dysfunction in the acute phase of stroke,and to explore the mechanism of the two methods.And provide a basis for its clinical promotion.Methods:According to the inclusive criteria and exclusive criteria,76 patients with dysphagia in the acute stage of stroke were selected,then they would be divided into a treatment group and a control group by simple randomization randomly,each with 38 cases.The two groups were given corresponding basic treatment,including stroke routine treatment,conventional acupuncture treatment.The treatment group was treated with Jinjin and Yuye spotting bloodletting,and the control group was given cold stimulation therapy.Jin jin and jade 1 iquid puncture and bloodletting operation method is that the operator clamps the tongue with a clean gauze and pulls out,and punctures the Jinjin and jade liquid with a 0.30mm×0.40mm disposable sterile acupuncture needle(Dongbang acupuncture needle)to make blood flow The amount of bleeding on one side was dipped in 2 cotton swabs,3 times a week,2 weeks for 1 course,a total of 2 courses.The method of cold stimulation therapy is that the operator takes out pure water at about 6℃ and puts it in a clean container.The dry cotton swab is dipped in pure water to gently stimulate the throat wall,tongue base,palatal arch and soft palate,and instruct the patient to swallow empty,once a day,each training time is about 10 minutes,2 weeks is a course of treatment,a total of 2 courses.Watian drinking water test and Gugging swallowing function assessment scale were used to evaluate the efficacy before and after treatment and 4 weeks after treatment.Collect relevant data and apply SPSS 21.0 statistical software to statistical analysis of the research data.Results:1.Baseline data:Comparing the basic information of age,sex,and disease duration between patients in the experimental group and the control group,there wasn’t significant difference between the data groups(P>0.05).The comparison of the Watian drinking water test and the GUSS score showed that there was no significant difference between the two scales(P>0.05).Comprehensive analysis showed that the baseline of the two groups was balanced,and the patients’conditions were comparable.2.Watian drinking water test:After statistical analysis,the Watian drinking water test ratings of the treatment group and the control group were lower than before treatment,indicating that both of the groups of swallowing disorders were improved,the difference was statistically significant(P<0.05).After the treatment in the group,the decrease in the rating of the Watian drinking water test was found to be more significant,and the difference had statistical significance(P<0.05),indicating that the Jinjin and Yuye point puncture bloodletting therapy was more effective in improving swallowing disorders.At the same time,the ratings of the Watian drinking test were compared after treatment and 4 weeks after the end of treatment,and it was found that the ratings of the two groups rose after 4 weeks after treatment,and there wasn’t a significant difference(P>0.05).3.GUSS score:After statistical analysis,the GUSS scores of the treatment group and the control group were lower than before treatment,indicating that swallowing disorders were improved in both groups,and there was significant difference(P<0.05).With the comparison of the two groups,the GUSS scores increased after treatment.It was found that the increase in the treatment group was more obvious,indicating that Jinjin and Yuye point puncture bloodletting therapy was more effective in improving swallowing disorders.At the same time,the GUSS scores were compared after treatment and 4 weeks after the end of treatment.It was found that both of the two groups’ scores were decreased after 4 weeks of treatment,and the decrease of the control group is greater than that of the treatment group.The difference had statistical significance(P<0.05).It shows that the effect of cold stimulation therapy decreases more significantly 4 weeks after the end of treatment.4.Comparison of efficacy:There had 13 cases(34.21%)cured,15 cases(39.47%)were markedly effective,6 cases(15.79%)were effective,4 cases(10.53%)were ineffective,and the rate of the total effective was 89.47%.5 cases were cured in the control group(13.16%),markedly effective in 8 cases(21.05%),effective in 15 cases(39.47%),and ineffective in 10 cases(26.32%).There was a significant difference in clinical efficacy between the two groups after treatments(P<0.05).Conclusion:Both Jinjin Yuye point bloodletting and cold throat stimulation can improve swallowing dysfunction after stroke.However,Jinjin Yuye point bloodletting has better efficacy than cold throat stimulation,and the effect of Jinjin Yuye point bloodletting treatment lasts longer.Therefore,as a simple,effective and safe treatment method,Jinjin jade liquid spotting and bloodletting therapy has certain advantages for the treatment of dysphagia after stroke,and is a worthy of clinical promotion. |
