Objective:to understand the research status of postoperative care dependence,self-care ability and quality of life of patients with gynecological malignancies;to compare the differences in general demographic characteristics of postoperative care dependence,self-care ability and quality of life of patients with gynecological malignancies:to analyze the correlation between postoperative care dependence,self-care ability and quality of life of patients with gynecological malignancies;to explore the relationship between postoperative care dependence,self-care ability and quality of life of patients with gynecological malignancies In order to provide theoretical guidance for nursing staff to formulate effective intervention measures.the influence of patients’ care dependence and self-care ability on the quality of life was analyzed. Methods:in this study, convenient sampling method was used to select the patients who met the inclusion criteria and exclusion criteria in three third class A hospitals in Jilin Province.Based on the number of patients who received gynecological malignant tumors in one year in three hospitals.320 questionnaires were sent out in the proportion of 2:2:1,302 of which were valid questionnaires, and the effective recovery rate was 94.4%.The research tools were general social demographic questionnaire, care dependence scale(CDS),self-care ability scale(ESCA).quality of life scale(EORTC-QLQ-C30).After the questionnaire is recovered.the data is input into excel table for sorting.and then imported into SPSS20.0 statistical software for statistical analysis of the original data. The statistical methods used in this study include descriptive,t-test.anovo one-way ANOVA or non parametric test.Pearson correlation analysis, multiple lineal regression analysisResults:(1)In this study, the total score of care dependence of patients with gynecological cancer after operation was(57.54±8.1 7).The patients were in partial dependence level as a whole. The total score of physiological dimension was(35.67 ± 5.89),The total score of physiological dimension was(21.86±4.17).37.5%.54.6%and 7.90%of them were mild.moderate and severe. There were significant differences in age.marital status.education level.residence.famil)per capita monthly income, work status, operation mode.postoperative days and main caregiver factors in the postoperative care dependence of gynecological cancer patients(P<0.05)(2)In this study, the total average score of self-care ability of patients with gynecological malignant tumor after operation was(110±14.29),the overall self-care ability of patients was in the middle level, the score of self-concept dimension was(20.06± 3.45),the score of self-care responsibility dimension was(16.81±2.80),The score of energy dimension is(30.15±4.45),the score of health knowledge level is(42.99±7.41).The self-care ability level is 0 in the low level,194 in the middle level, accounting for 64.2%of the total number,108 in the high level.accounting for 35.8%of the total number. There were significant differences in the scores of patients’ self-care ability in age, marital status, education level, family per capita monthly income, working status, payment mode, hospitalization experience.treatment type, postoperative days and main caregivers(P<0.05).(3)In this study, the quality of life score of patients with gynecological cancer after operation was(57.97 ± 19.12),and the overall quality of life of patients was at a low level. The scores of physical function dimension is(71.30 ± 1 7.80),the role function dimension is(63.74±25.74),the emotional function dimension is(78.31 ±18.11),the social function dimension is(66.58±17.14).the fatigue subscale is(59.93 ± 21.67)and the pain subscale is(35.58±18.83).respectively The scores were(25.05 ± 16.79),nausea and vomiting(32.84±22.71),dyspnea(10.26 ±15.89)insomnia(30.13±23.86),appetite(38.41±22.82).constipation(26.05 ± 21.70).diarrhea(11.48 ±18.65).economic difficulties(37.42±27.61).There were significant differences in the scores of life quality in age.marital status.education level.family per capita monthly income.payment method.treatment type and main caregivers(P<0.05)(4)In this study.there was a negative correlation between the degree of care dependence and the ability of self-care(P<0.05):a negative correlation between the degree of care dependence and the quality of life(P<0.05):a positive correlation between the ability of self-care and the quality of life(P<0.05)(5)A fter multiple linear regression analysis of the influencing factors of patients’ quality of life.this study concluded that the main influencing factors of the quality of life of patients with gynecological malignant tumor were treatment type.self-concept.self-care responsibility.self-care skills and health knowledge level.Conclusions:(1)the degree of dependence on care and the ability of self-care of patients with gynecological malignant tumor after operation are in the middle level, and the quality of life is in the lower level.(2)there was correlation among the nursing dependence,self-care ability, quality of life and all dimensions of patients with gynecological cancer after operation. That is to say,the lower the patient’s dependence on care, the higher the self-care ability and the better the quality of life(3)the main influencing factors of the quality of life of patients with gynecological cancer after operation are the type of disease treatment,self-care ability(self-concept,self-care responsibility,self-care skills, health knowledge level).Nursing dependence and self-care ability can better predict the quality of life of patients with gynecological cancer after surgery. There is a significant correlation between the three variables. In the future clinical work, medical workers should encourage patients to reduce their dependence on others, improve their self-care ability through corresponding interventions.so as to improve the quality of life of patients.
