| Objective:To study the clinical effect of Zhitong Tiaoxue Recipe on preventing recurrence of ovarian endometriosis(EMT),to explore its mechanism,and to discuss the correlation between serum CA125,VEGF and EMT and recurrence after operation.Methods:Two groups of Chinese and Western medicine were randomly divided into two groups.The subjects were given medicine on the fifth day after conservative surgery for three months.The traditional Chinese medicine group was given Zhitong Tiaoxue recipe,while the western medicine group was given Noride.Observe and record the dysmenorrhea,menstruation,gynecological examination of the two groups,and parallel pelvic B ultrasound,serum CA125,vascular endothelial growth factor examination.The symptoms,signs,amenorrhea rate,recurrence rate,adverse reaction rate and pregnancy rate were observed and compared between the two groups.Results:(1)Dysmenorrhea: After 3 months of medication and 3 months of withdrawal,the degree of dysmenorrhea and symptom scores of the two groups were better than those before operation,P < 0.05,indicating that both groups of medication can better alleviate the degree of pain and improve the symptoms of dysmenorrhea.(2)Menstruation: During medication and 3 months afterwithdrawal,the incidence of amenorrhea was zero in the Chinese medicine group,100%(30/30)in the western medicine group and 6.67%(2/30)in the three months after withdrawal,indicating that the Chinese medicine group did not inhibit ovarian function.(3)Gynecological examination: 3 months after medication and 3 months after withdrawal,the scores of gynecological examination of the patients were significantly higher than those before operation(P < 0.05).The gynecological examination of the two groups were significantly improved compared with those before operation.(4)Serum CA125 and vascular endothelial growth factor: 3 months after withdrawal,the blood values of serum CA125 and vascular endothelial growth factor decreased compared with those before operation,P < 0.05,indicating that both groups of drugs can reduce the levels of CA125 and vascular endometriosis,which has a certain correlation with the severity of endometriosis.There was no significant difference between the two groups(P > 0.05).(5)Recurrence rate: 9 months after drug withdrawal,pelvic B-mode ultrasound showed abnormal echo mass in the adnexal area.There were 2 cases in the traditional Chinese medicine group,with a recurrence rate of 6.67%(2/30),and 4 cases in the western medicine group,with a recurrence rate of 13.33%(4/30).The comparison between the two groups,P > 0.05,showed that the two groups had the same effect in preventing the recurrence of endometriosis after operation,and the recurrence rate of the traditional Chinese medicine group was slightly lower.(6)The incidence of adverse reactions was 10%(3/30)in traditional Chinese medicine group and 43.33%(13/30)in western medicine group.There was significant difference between the two groups(P < 0.05).The incidence of adverse reactions in the traditional Chinese medicine group was significantly lower than that in the western medicine group.(7)Pregnancy rate in 1 year after operation: 33.33%(5/15)in Chinese medicine group and 20.00%(2/10)in western medicine group.The pregnancy rate in the Chinese medicine group was slightly higher than that in the western medicine group,but there was no significant difference betweenthe two groups(P > 0.05).Conclusion: The clinical effect of Zhitong Tiaoxue Prescription in preventing recurrence after EMT is reliable.The prescription can improve the symptoms and signs of dysmenorrhea.It is similar to Noride in reducing the expression of serum CA125 and VEGF and preventing recurrence after EMT.The incidence of amenorrhea,recurrence and adverse reactions was low,and the ovarian function was not inhibited.The pregnancy rate was relatively high one year after operation. |
