Objective:Understand the distribution characteristics of plasma homocysteine(Hcy)levels in premenopausal and postmenopausal women and their correlation with sex hormones,blood sugar,blood lipids and other indicators,then explore the influencing factors of female hyperhomocysteinemia(HHcy)to further discuss the impact of HHcy on cardiovascular diseases(CVDs)in women,hence provide new and valuable monitoring indicators for the prevention of cardiovascular diseases in perimenopausal women.Methods:(1)Performed retrospective study on medical records of 622 participants aged 40-69 years who were treated for benign diseases in the gynecology department at the Affiliated Hospital of Hangzhou Normal University from January 2018 to December 2019.(2)Collected general demographic characteristics,Hcy,sex hormone levels,lipid levels and fasting blood glucose detection indicators of all the study respondents.(3)Used SPSS 25.0 software for data analytics.Applied Spearman analysis to analyze the correlation between Hcy and related indicators;applied Logistic regression model to analyze the influencing factors of HHcy for women.Result:(1)For a total of 622 study participants whose ages ranged from 40 to 69 years,the average age of participant was(49.51±6.76)years.The median Hcy level was 9.60(7.70,12.00)μmol/L,and the incidence of HHcy was 43.73%.The patients were divided into two sets of groups.For one set,159 patients were in the premenopausal group and 463 patients were in the postmenopausal group.The Hcy level in the postmenopausal group was 10.70(8.80,12.90)μmol/L,which in the premenopausal group was 9.30(7.40,11.30)μmol/L,and the difference was statistically significant(P<0.05);The incidence of HHcy in the postmenopausal group was 58.5%,which in the premenopausal group was 38.6%,and the difference was statistically significant(P<0.05).For another set,272 patients were in the HHcy group and 350 patients were in the normal Hcy group.the average age in the Hcy group was(48.39±6.14)years,which in the HHcy group was(50.96± 7.23)years,and the difference is statistically significant(P<0.05);in the HHcy group,the E2 level was reduced,while FSH level and LH level were increased than which in the Hcy normal group,and both sets of difference were statistically significant(P<0.05).(2)Spearman correlation analysis showed that Hcy was positively correlated with age(r=0.198,P<0.05),FSH(r=0.188,P<0.05)and LH(r=0.193,P<0.05),while negatively correlated with T(r=-0.110,P<0.05)and E2(r=-0.130,P<0.05),and not correlated with TC,TG,LDL-C,HDL-C.The correlation between HHcy and age,sex hormones,and biochemical parameters was analyzed using the Kendall’stau-b correlation test,and the results showed that the occurrence of HHcy was positively correlated with age(r=0.149,P<0.05),FSH(r=0.158,P<0.05),LH(r=0.162,P<0.05)and HDL-C(r=0.066,P<0.05),while negatively correlated with T(r=-0.670,P<0.05)and E2(r=-0.870,P<0.05).(3)First,univariate logistic regression analysis was applied,by using HHcy occurrence as the dependent variable,age,menopausal status,sex hormone levels,blood glucose,and blood lipids as independent variables.The results showed that statistically significant independent variables included age(P<0.05),menopausal status(P<0.05),E2(P<0.05),LH(P<0.05)and FSH(P<0.05).Afterwards,the independent variables age,menopausal status,E2,LH and FSH were included in multivariate Logistic regression,The results showed that women with FSH≥40 IU/L had a 2.01-fold higher risk of HHcy than women with FSH<40 IU/L(Adjust OR=2.01,95%Cl:1.14-3.54).Conclusion:(1)Postmenopausal women’s Hcy level and the incidence of HHcy were increased compared to which of premenopausal women;Hcy is positively correlated with age,FSH,LH,while negatively correlated with T,E2,and not correlated with TC,TG,LDL-C,HDL-C.HHcy is positively correlated with age,FSH,LH,HDL-C,while negatively correlated with T and E2.(2)The increased FSH level in postmenopausal women may be one of the risk factors leading to the increasing of Hcy level.Including the monitoring of Hcy level in the screening items for perimenopausal women may enhance the prevention and treatments of cardiovascular diseases,to a certain extent.
