| Objecti ve:To observe the effect of Badushengji powder combined with compound polymyxin B ointment（hereinafter referred to as Funuo）on the wound healing of diabetes foot ulcer,and to explore the clinical efficacy of Badushengji powder.Methods:60 patients with diabetes foot ulcer（DFU）admitted to the wound repair and peripheral vascular Department of Wuhan integrated traditional Chinese and Western Medicine Hospital（hereinafter referred to as our department）from September 1,2020 to September 1,2021 were randomly divided into control group and treatment group.There were 30 cases in each group.On the basis of internal treatment such as controlling blood glucose,controlling infection,improving circulation,nourishing nerves,promoting blood circulation and removing blood stasis,the patients in the two groups were evaluated to have surgical indications and no surgical contraindications.After receiving debridement treatment in the operating room,the wound dressing was routinely changed in the medicine changing room of our department,and the wound was washed and disinfected with Iodophor,3%hydrogen peroxide（hydrogen peroxide）disinfectant and 0.9%sodium chloride injection,The control group was treated with compound polymyxin B ointment and medical iodophor solution gauze,and the treatment group was treated with Batu Shengji powder on the basis of the control group.Once every other day for 3 weeks.The changes of subjective feeling（pain,numbness,chills,dry skin,lameness）,overall curative effect（total effective rate）,ulcer healing area and ABI index were compared between the two groups.Results:Among 60 patients with diabetes foot,18 were cured,35 were markedly effective and effective,and the effective rate was 88.33%.After treatment,the improvement rates of pain,numbness,chills,dryness and lameness were 96.49%,98.25%,94.74%,92.98%and 96.49%,respectively.There were no amputations and deaths,and 3 cases of abscission（including 1 case in the treatment group,which was excluded because the patient could not change the dressing regularly;2 cases in the control group,which was excluded because the patient’s symptoms improved during the dressing change in hospital and strongly asked to leave the hospital）.The total course of treatment was 3 weeks.There was no significant difference in liver and kidney function,blood glucose and blood lipid between the two groups after treatment（P>0.05）,but they were lower than those before treatment.Comparison of overall treatment effect（total effective rate）and ulcer healing area after treatment:there was significant difference between the two groups before and after treatment（P<0.05）,and the treatment group was better than the control group（P<0.05）.After 3 weeks of treatment,the main symptoms（degree of pain,numbness,chills,dry skin,etc.）of the two groups were improved.The treatment group was significantly improved compared with the control group（P<0.05）,and there was no significant difference in ABI index（P>0.05）,but they were improved.Conclusions:（1）The treatment of DFU with Badu Shengji powder combined with Funuo can significantly improve the symptoms and signs（pain,numbness,chills and dry skin）,promote the growth of fresh granulation,achieve the purpose of wound healing,reduce the area of ulcer and improve the level of ABI index,so as to improve the overall cure rate,reduce the length of hospital stay and save the cost of hospitalization.（2）Comparison of blood glucose and lipid metabolism between the two groups before and after treatment:the blood glucose metabolism and lipid metabolism of the two groups after treatment were lower than those before treatment,and there was no significant difference between the two groups after treatment（P>0.05）,indicating that the treatment effect of the treatment group on glucose metabolism and lipid metabolism was the same as that of the control group,and the treatment of the two groups was effective.（3）Comparison of the overall treatment effect（total effective rate）between the two groups before and after treatment:the total effective rate of the treatment group was 96.55%,and that of the control group was 89.29%.By rank sum test,the overall efficacy of the two groups before and after treatment was compared（P<0.05）,which was statistically significant,indicating that the treatment of the two groups was effective.（4）There was significant difference in TCM syndrome scores between the two groups before and after treatment（P<0.05）;The comparison of TCM syndrome scores between the two groups was statistically significant（P<0.05）,suggesting that both groups improved after treatment,and the treatment group was better than the control group.（5）After treatment,the main symptom scores of the two groups were compared within and between groups,and the difference was statistically significant（P<0.05）,suggesting that the two groups were improved after treatment,and the treatment group was better than the control group.（6）By t-test,there was no significant difference in ABI between the groups before and after treatment（P>0.05）.There was no statistically significant difference between the two groups after treatment（P>0.05）,but they all improved.The patients in this study had no adverse reactions and had good curative effect.It shows that single use of Funuo and Badushengji powder combined with Funuo are effective in the treatment of diabetes foot ulcer,and the combined use of Funuo has significant effect,safe medication,convenient operation,strong patient compliance,and is easy to accept the treatment scheme. |
