| Objectives A retrospective study was conducted to compare the differences in operation time,blood loss,postoperative function,fracture healing time and average hospitalization cost in patients with simple olecranon fracture treated with locking plate and Kirschner wire tension band.Analyze the treatment effect and cost of the two,and find out the most effective-cost treatment scheme,in order to provide some reference for the treatment of simple olecranon fracture in clinical practice.Methods The data of patients with Mayo ⅠA and Mayo ⅡA olecranon fractures treated with Kirschner wire tension band or locking plate in the Affiliated Orthopaedic Hospital of North China University of Science and Technology from October 2013 to August 2019 were collected and calibrated as the target of the study.through the screening of inclusion criteria and exclusion criteria,a total of 65 patients were obtained,including medical records,various imaging data,surgical procedures,etc.The data obtained were recorded and sorted out for retrospective statistical analysis.According to the choice of internal fixation,65 patients with simple olecranon fracture were divided into two groups.The patients in group A were treated with Kirschner wire tension band and the patients in group B were treated with locking plate.There were 30 patients in group A,male / female: 14/16,mean age: 41.97 ±14.61 years,left / right: 16/14,injury factors: fall in 20 cases,traffic injury in 6 cases,sports injury in 4 cases.The time from injury to operation was 3.13 ±1.20 days.According to Mayo classification,there were 3 cases of type ⅠA and 27 cases of typeⅡA.A total of 35 patients in group B,male / female: 14 /21,mean age: 45.03 ±15.34 years old,left / right: 16 /19,injury factors: fall injury 25 cases,traffic injury 5 cases,sports injury 5 cases.The time from injury to operation was 3.43 ±1.38 days.According to Mayo classification,there were 5 cases of type ⅠA and 30 cases of type ⅡA.First of all,the baseline data of patients in group An and group B were compared,and after determining the comparability,the data of operation time,incision length,intraoperative blood loss,fracture healing time,range of motion of elbow,excellent and good rate of Mayo elbow function score,average hospital stay,average hospitalization cost and postoperative complications were collected and analyzed by statistical software.To compare the therapeutic effect and cost of simple olecranon fracture with locking plate and Kirschner wire tension band fixation.Results All patients participated in the follow-up completely,and the follow-up time was7-27 months,with an average of 15.26 ±4.36 months.The average fracture healing time was 10.72 ±1.52 weeks in group A and 10.96 ±1.67 weeks in group B.there was no significant difference in fracture healing time between the two groups(P>0.001).There were significant differences in average operation time,incision length and blood loss(P<0.05).There was no significant difference in VAS pain score and range of motion of elbow joint between the two groups during the whole follow-up period,and there was no significant difference in the average hospitalization days between the two groups,but in the average hospitalization expenses: 14980 ±1791.38 yuan in group An and 24479.17±2284.54 yuan in group B(P<0.001).There was a significant difference between the two groups;the incidence of complications in group A was 33.3%.The incidence of complications in group B was 11.4%,and the difference was statistically significant(P<0.05).According to the Mayo elbow function score,the excellent and good rates of group An and group B were 93.3% and 88.6%,respectively.Conclusions In the treatment of simple olecranon fracture,the operation time,incision length,intraoperative blood loss and hospitalization cost in the Kirschner wire tension band group were lower than those in the locking plate group,but there was no difference in fracture healing time,average hospital stay,elbow VAS score and joint function recovery.The incidence of complications in the Kirschner wire tension band group was higher than that in the locking plate group.In the treatment of simple olecranon fracture,both of them are effective ways to treat simple olecranon fracture.Figure 18;Table 8;Reference 106... |
