| Objective To compare the tension band, olecranon anatomical plates, anatomic locking plate three methods of treating Type II Mayo olecranon fractures, provide theoretical and practical basis for the clinical treatment of olecranon fracture fixation method to select the best and initially grasp the basic ideas and methods of clinical research through the research process.Methods From 2010 January to 2013 May in Department of orthopedics Affiliated Hospital of North China University of science and Technology(the second hospital of Tangshan) clinical data ofolecranon fracture trauma department were selected, with Kirschner wire tension belt,olecranon anatomical plate, anatomical locking plate of three methods in the treatment of a total of 98 cases of Mayo type II fracture of the olecranon cases, the relevant cases ofoperation, data, image data and follow-up data were collected for retrospective statistical analysis. 98 patients received 13~24 months follow-up, an average of 19.5 months, one year after the operation of the elbow joint function were recorded, the modified Mayo elbow performance score for functional evaluation. According to the different fixation methods are divided into three groups, one Kirschner wire tension band group(A group), 35 cases; anatomical plate group(B group) 32 cases; anatomical locking plate group(C group) 31 cases; the Kirschner wire tension band group male 16 cases, female 19 cases, age 21~65 years old, average 38.3 years old; anatomical plate group male 18 cases, female 14 cases, age 19~65 years old, average 40.9 years old, locking plate group male 12 cases, female 19 cases, age 21~58 years old, average 41.5 years old; cause of injury: 27 cases of traffic accident, falling injury in 18 cases, female 11 cases, against injury in 11 cases, injuries in 31 cases. The fracture type: Mayo type IIA 49 cases, Mayo 49 cases of type IIB. All case of illness all belong to closed fracture. Of all the medical records of the operation, the fracture healing, functional recovery exercise time, postoperative complications and postoperative excellent rate was used for statistical analysis, comprehensive comparison of the three methods.Result Three groups in operation time, fracture healing time,bleeding of operation compared had no statistically significant(P>0.05), there was no difference between three groups.The comparison of three groups in the functional rehabilitation exercise time, postoperative recovery of elbow joint function excellent rate, complication rate comparisonhad no statistically significant(P<0.05).The comparison of the incidence rate of locking plate group was better than that of anatomical plate and Kirschner wire tension band group in functional rehabilitation exercise time, the excellent and good rate, complications, between the anatomic plate and Kirschner wire tension band group had no difference.Conclusion Three kinds of internal fixation methods for the treatment of Mayo type II are effective methods for olecranon fractures, locking plate began functional exercise time, elbow joint function score and incidence of postoperative complications was better than Kirschner wire tension band and anatomical plate group after the operation, and anatomical plate group was no significant difference in efficacy of Kirschner wire tension belt in the process of clinical treatment, the physical condition of patients considered for Mayo type II fracture of the olecranon suggested that the application of locking plate fixation of olecranon. |
