| Objective: To observe the level of serum mi R-497,interleukin 6(IL-6),tumour necrosis factorα(TNF-α)and Vascular Endothelial Growth Factor(VEGF)in healthy people and type 2 diabetes mellitus(T2DM) patients with or without diabetic retinopathy(DR)and analyzed the co--rrelation between mi R-497 and glucose and lipid metabolism,inflammat ory factors,vascular endothelial growth factor and other indicators to ex plore the relationship between mi R-497 and DR.Methods: The 120 subjects who were diagnosed with type 2 diabe tes and adimited to our hospital from August 2019 to November 2020 were selected.According to the presence of retinopathy,they were divid ed into retinopathy group(DR group)and simple T2 DM group(T2DM group).Diabetic nephropathy,diabetic peripheral neuropathy,cardio-cere brovascular disease and lower limb vascular disease were excluded in th e single T2 DM group.The 60 healthy subjects were enrolled as the hea lthy control group(NC group)during the same period.General data we re collected such as medical history,medication history and past history.Glucose,blood lipids,renal function,glycosylated hemoglobin(Hb A1c)and other indicators were detected.Serum interleukin 6(IL-6),tumor ne crosis factor α(TNF-α),vascular endothelial growth factor(VEGF)level s were detectded by ELISA.And Serum mi R-497 was detected by rever se transcription real-time fluorescence quantitative(RT-PCR).Results:1.Comparison of clinical indexes among NC group,T2 DM group a nd DR group.There was no statistical difference in terms of age,gender,smoking,alcohol,and the level of total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),and uric acid(UA)among the three groups(P>0.05).Body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),triglyceride(TG),fasting blood glucose(FBG),urea nitrogen(BUN),Hb A1 c,insulin resistance index(HOMA-IR)in T2 DM groups and DR groups were higer than those in NC groups(P<0.05),While glomerular filtration rate(e GFR),high density lipoprotein cholesterol(HDL-C)in T2 DM groups and DR groups were lower than those in NC groups(P<0.05).Howere,there was no significant difference in the above indexes between the T2 DM group and the DR group(P<0.05).The duration of diabetes and 24-hour urinary microalbumin(UMA)in DR group were significantly higher than those in T2 DM group(P < 0.05),And there was no significant difference in 24-hour urinary protein level between the two groups(P>0.05).2.The levels of IL-6,TNF-α,VEGF and mi R-497 among the three groupsSerum mi R-497 level [5.43(3.23,8.36)vs 2.78(0.59,6.32)vs 1.61(0.56,3.68)]showed a gradual decrease in the NC group,T2 DM group and DR group,and the difference between the groups was statistically significant(P<0.05).Serum IL-6 [5.59(3.29,9.85)vs 9.53(6.37,13.6)vs 11.53(9.19,14.33)pg/ml],TNF-α [8.01(5.38,13.21)vs 15.93(11.05,35.97)vs26.4(18.98,40.46)pg/ml] and VEGF [233.13(168.08,387.59)vs 360.12(211.88,603.22)vs 549.46(345.08,687.86)pg/ml] expression levels showed an increasing in the NC group,T2 DM group,and DR group,and the difference between the groups was statistically significant(P<0.05).3.Serum mi R-497 was negatively correlated with FBG,Hb A1 c,HOMA-IR,TG,UMA,IL-6,TNF-α,VEGF,and positively correlated with HDL-C,There was no significant correlation with the course of diabetes,BMI,TC,LDL-C,Cr,BUN,UA,e GFR.VEGF,IL-6,and Hb A1 c were independent influencing factors of serum mi R-497.4.mi R-497 was a protective factor for diabetic retinopathy.the course of diabetes and 24-hour urine microalbumin were risk factors for diabetic retinopathy.Conclusions:1.Serum mi R-497 level decreased in patients with DR.2.VEGF,IL-6,and Hb A1 c were independent influencing factors of serum mi R-497,suggesting that mi R-497 may be involved in inflammatory pathways and neovascularization to affect the progression of DR.3.Mi R-497 was a protective factor for diabetic retinopathy.The course of diabetes and 24-hour urine microalbumin were risk factors for diabetic retinopathy. |
