Objective: Detect the concentrations of VEGF and -IL-6 in the aqueous humor and plasma of type 2 diabetic patients. To explore the changes of the concentrations of VEGF and IL-6 in the aqueous humor and plasma and to investigate the roles of VEGF and IL-6 in the pathogenesis of diabetic retinopathy (DR) and the clinical meaning .Methods: The concentrations of VEGF and IL-6 in the plasma and aqueous humor were determined by the enzyme-linked immunosorbent assay (ELISA) technique. The plasma and aqueous humor concentrations of VEGF and IL-6 were measured in 64 type 2 diabetes patients with or without retinopathy and 20 normal subjects . DM patients were divided into three groups according to dilated fundus examination and fluorescein angiography and fundus photograph:21 type 2 diabetes patients without retinopathy (NDR), 25 with bachground diabetic retinopathy (BDR),18 with proliferative diabetic retinopathy (PDR).The control group were 20 healthy patients with senile cataract.Results: The concentrations of VEGF in the aqueous humor samples from the diabetic patients without retinopathy (NDR),bachground diabetic retinopathy (BDR) and proliferative diabetic retinopathy (PDR) were (215.05±30.33 ) pg/ml,(296.75±46.92) pg/ml,(335.3±231.52) pg/ml; The concentrations of IL-6 in the aqueous humour from NDR,BDR and PDR were(128.17±25.25) pg/ml,(252.18±49.88)pg/ml,(288.10±35.22)pg /ml;The concentrations of VEGF and IL-6 in the aqueous humour from the control group were(192.73±39.57) pg /ml,(97.45±21.48) pg /ml. The concentrations of VEGF in the plasma samples from NDR,BDR,PDR were (206.75±31.51) pg /ml,(283.47±59.27) pg /ml,(459.07±90.90) pg /ml; The concentrations of IL-6 in the plasma samples from NDR,BDR,PDR were (119.21±23.81) pg/ml,(218.63±64.72) pg/ml,(387.95±90.22) pg/ml ;The concentrations of VEGF and IL-6 in the plasma from the control group were (135.50±25.69) pg/mk (78.71±20.37) pg/ml. Compared with the control, the concentrations of VEGF and IL-6 in the plasma samples of diabetic patients increased significantly (x~2==36.32,P<0.01; x~2=43.31,P<0.01) . Compared with the control, the concentrations of VEGF and IL-6 in the aqueous humor of diabetic patients increased significantly (x~2=43.44,P<0.01; x~2=46.34,P<0.01) .The concentrations of VEGF and IL-6 in the plasma and aqueous humour samples from the NDR,BDR,PDR all increased significantly. The concentrations of VEGF in the plasma samples was significantly correlated with the concentrations of IL-6 in the plasma samples (x~2=43.44,P<0.01) . The concentrations of VEGF in the aqueous humor samples was significantly correlated with the concentrations of IL-6 in the aqueous humor samples (x~2=46.34,P<0.01) . The concentrations of VEGF and IL-6 in the plasma samples was significantly correlated with the severity and prolongation of DR (rs =0.423,P<0.01; r s=0.744,P<0.01),(r_s=0.506,P <0.01;r-s =0.792, P <0.01) . The concentrations of VEGF and IL-6 in the aqueous humour was significantly correlated with the severity and prolongation of DR (r_s=0.480,P<0.01; r_s=0.824,P <0.01 X (r_s =0.482,P <0.01;r_s=0.856, P <0.01 ).The concentrations of VEGF and IL-6 in the plasma from the control group,NDR,BDR were higher than the concentrations of VEGF and IL-6 in the aqueous humor,but the concentrations of VEGF and IL-6 in the plasma from the PDR were lower than the concentrations of VEGF and IL-6 in the aqueous humor.Conclusion:1. VEGF and IL-6 plays an active role in the generation and development of DR. With the prolongation of DM and the severity of DR, the concentrations of VEGF and IL-6 in the plasma and aqueous humour all increased. 2. With the prolongation of DM and the severity of DR,the concentrations of VEGF and IL-6 in the aqueous humour were higher than the concentrations of VEGF and IL-6 in the serum,think that there is a local mechnism in ocular about VEGF and IL-6.3. The concentrations of VEGF have a close relation with the concentrations of IL-6 in the plasma and aqueous humour from the diabetic patients. Our results suggest that there is a significant relationship between VEGF and IL-6 in the in the generation and development of DR. Measurement of the aqueous levels of VEGF and IL-6 may be useful to analyze the pathogenesis of DR.
