Research On Operation Effect Of Regional Medical Group

Purpose: The establishment of regional medical groups is an important measure to solve the problems of division of medical and health care and health services are fragmented in China.this study takes a medical group in Ning de as an example,through the methods of questionnaire and interview,investigation,collect medical group four members to the hospital,the high quality of micro data on the effect of the reform of the hospital medical group members and operation mode analysis,the development of medical group of subject research,think about the opportunities and challenges faced in the process of reform,put forward policy suggestions to provide a more scientific basis for the formulation and improvement of national and Fujian provincial related policies.Methods: Through literature analysis,the development status and related basic concepts of medical groups at home and abroad were clarified,and the reform situation of regional medical groups was familiar.37 specific indicators in 8 categories were taken as the evaluation indexes of the effect before and after the reform of medical groups.the cooperative management theory is used to sort out the development status and reform measures of a medical group in ningde,and the function coupling of each member hospital of the medical group and its various elements is carried out to form a cooperative governance structure,cooperative development mechanism and cooperative management mechanism.the survey data came from the financial statistical statements of county hospital A,county hospital B,county hospital C and county hospital D from2010 to 2018,the evaluation scale before and after the reform of the member hospitals of the medical group,the survey of medical staff satisfaction and patient satisfaction.the changes and operation results of the four member hospitals of the medical group before and after the reform descriptively analyzed.through interviews with relevant principals of core hospitals and member hospitals,the reform situation and resistance factors of the medical group were understood,the semi-tight type and loose type were compared,the problems existing in the implementation process of the medical group were discussed,and policy suggestions were put forward.SPSS19.0 statistical software was used to process the satisfaction data.the reform effectiveness of each member hospital of the medical group was mainly represented by the original value and its growth rate and annual average growth rate of each index;the satisfaction of medical personnel and patients was represented by the constituent ratio;Ridit analysis was used to compare the indicators between groups.Results: The reform of the member hospitals of the medical group has achieved periodic results,after each index reform is obviously better than before the reform: the hospital scale expands unceasingly,business blowout development;rapid growth in total assets;the total number of workers,medical staff income increased significantly;the capacity and quality of medical services have improved significantly;urban and rural medical insurance out-of-pocket ratio and drug proportion gradually decreased,reduced burden of medical treatment;overall satisfaction of medical staff in 4 member hospitals was 64.27%,the medical staff’s satisfaction with the changes before and after the reform was 65.20%,the job identity satisfaction 62.88%,the workload satisfaction61.95%,the promotion evaluation satisfaction 54.76%,the salary system satisfaction51.04%,the holiday subsidy satisfaction 47.10%;patients’ overall satisfaction with 4member hospitals was 87.21%,the satisfaction degree of medical treatment environment was 85.42%,the satisfaction degree of service facilities was 81.84%,the satisfaction degree of service attitude of medical staff was 91.56%,and the satisfaction degree of medical technology was 80.05%.the results show that the reform of semi-tight type has achieved remarkable results,loose typ followed.therefore,the establishment of medical groups in different regions should be based on the local regional health planning,fully consider the nature and actual situation of different medical institutions,and select the corresponding mode to establish medical groups in different stages of development.Conclusions: There are some problems in the operation of medical group,such as loose cooperation mode,low efficiency of medical resource integration,low total amount of medical resources,affecting the improvement of medical technology and service ability,and some difficulties in two-way referral.the average cost has increased and the satisfaction of medical personnel and patients needs to be further improved.in order to promote the coordinated development of the regional medical groups and benefit the people of mindong,the researchers proposed the following policy suggestions: to promote close medical group,to build community of interest,to strengthen the efficiency of medical resource,to improve the level of medical technology and service capabilities,to improve the two-way referral mechanism,to promote the city,county and township level diagnosis and treatment system landing,to strengthen the fine management of medical groups,to improve people’s satisfaction and other policy recommendations.
